... اضطرابات النطق واللغة ...

مقدمة: ينظر إلى القدرة على التواصل على أنها مهارة أساسية في حياة المجتمعات الإنسانية وعنصر أساسي في تكوين إنسانية الإنسان وارتباطه بوسطة الاجتماعي ويقصد بالتواصل استقبال الرسائل والتعبير عن الذات اعتماداً على الكلام واللغة الكتابية، والأشكال المتنوعة من التواصل غير اللفظي، كاللغة الإرشارية، ولغة الجسم والعيوان.
واللغة في نهاية التحليل قدرة مكتسبة يتعلمها الطفل بالسماع والممارسة وبورح المتعة واللعب دون الحاجة إلى تعليم نظامي قبل مدة طويلة من ذهابه إلى المدرسة، ويعد اكتساب الطفل للغة المجتمع الذي يعيشه مظهراً طبيعياً لنضجه المبكر، فالأطفال يصغون إلى ما يقوله الكبار، ويقلدونه ويتعلمون الاستجابة له بحيث يكونون في حاولي الخامسة من العمر يكونون قد اكتسبوا نوعاً من اتفان اللغة يكفي لأن يكون أساساً لغوياً متيناً يمكن للمعلمين البناء عليه في تعليمهم للقراءة والكتابة.
إلا أن ثمة بعضاً من الأطفال يواجهون مصاعب خطيرة في محاولاتهم اكتساب اللغة والكلام، وبما أن اللغة جزءاً أصيلاً من الحياة العقلية، بمعنى أنها تحتاج إلى الإدراك والتذكر والإصغاء والاستيعاب والتفكير، فإن من الطبيعي أن الأطفال الذين يعانون مشكلات في لغتهم أو اضطراب في كلامهم، يواجهون عقبات في تحصيلهم الأكاديمي وفي كل مظهر وظيفي يقومون به.

وتقسم اللغة من حيث طبيعتها إلى مظهرين رئيسيين:
الأول: اللغة غير اللفظية (الاستقبالية) (Receptive Language)
الثاني: اللغة اللفظية (التعبيرية (Expressive Language).
ويرتبط بمفهوم اللغة (Language) مصطلحات أخرى مثل مصطلح الكلام (Speech) ومصطلح النطق (Articulation) إذ ترتبط هذه المصطلحات ببعضها البعض.
إذ يقصد بالكلام القدرة على تشكيل وتنظيم الأصوات في اللغة اللفظية، وأما النطق فيقصد به الحركات التي تقوم بها الحبال الصوتية أو جهاز النطق أثناء إصدار الأصوات.

مراحل تطوير اللغة:
يشير ماكندلس (Mecandlees, 1973) وفتزجيرلد (fitzgerald) إلى أن للغة أصولاً بيولوجية تتمثل في الاستعداد الفزيولوجي والعقلي، وأنها تسير وفق أربع مراحل هي:
1. مرحلة البكاء (Crying Stage): في هذه المرحلة يعبر الطفل عن حاجاته وانفعالاته بالصراخ وتمتد هذه المرحلة منذ الميلاد وحتى الشهر التاسع من العمر.
2. مرحلة المناغاة (Babbling Stage) وفي هذه المرحلة يصدر الطفل الأصوات أو المقاطع ويكررها، وتمتد هذه المرحلة من الشهر الرابع والخامس تقريباً وحتى الشهر الثامن والتاسع.
3. مرحلة التقليد (Imitation Stage): وفي هذه المرحلة يقلد الطفل الأصوات أو الكلمات التي يسمعها تقليداً خاطئاً، فقد يغير أو يبدل أو يحذف أو يحرف مواقع الحروف في الكلمات التي ينطقها، وقد يرجع ذلك إلى عوامل كثيره أهمها مدى نضج جهاز النطق، وضعف الإدراك السمعي، وقلة التدريب، ولكن مع استمرار عوامل النضج والتعلم والتدريب تصبح قدرة الطفل على التقليد أكثر دقة، وتمتد هذه المرحلة منذ نهاية السنة الأولى من العمر وحتى الرابعة أو الخامسة تقريباً.
4. مرحلة المعاني (Semantic Stage): في هذه المرحلة يربط الطفل ما بين الرموز اللفظية ومعناها، وتمتد هذه المرحلة منذ السنة الأولى من العمر وحتى عمر الخامسة وما بعدها.
تختلف مظاهر المحصول اللغوي للطفل من عمر إلى آخر، حسب مراحل نموه الزمني، إذ تعتبر السنة الأولى من عمر الطفل هي مرحلة الكلمة الواحدة، وتعتبر السنة الثانية مرحلة الجملة ذات الكلمتين وتعبر السنة الثالثة مرحلة تكوين الجمل، أما السنة الرابعة فتعتبر مرحلة تناول الحديث مع الآخرين أما الخامسة فهي مرحلة تكوين الجملة الكاملة.

العوامل التي يتأثر بها النمو اللغوي كما حددها فتزجيرلد (Fitzgerald, 1973) هي:
1. الجنس (Sex): إذ يلاحظ أن الإناث أسرع من الذكور في النمو اللغوي.
2. العوامل الأسرية (Familly factors): ويقصد بها ترتيب الطفل في الأسرة، والظروف الاقتصادية والاجتماعية للأسرة، فالطفل الوحيد في الأسرة أكثر ثراء في محصوله اللغوي مقارنة مع الأطفال العديدين. كما أن أطفال المؤسسات والملاجئ أقل محصولاً من الناحية اللغوية مقارنة مع الأطفال الذين يتربون في أسرهم بسبب قلة خبراتهم واتصالهم مع الآخرين وإهمالهم أحياناً.
3. الوضع الصحي والحسي للفرد (physical –sensory position ) ويقصد بذلك أهمية الجوانب الصحية والحسية للفرد وعلاقتها بالنمو اللغوي، إذ يتأثر النمو اللغوي بسلامة الأجهزة الحسية السمعية والبصرية والنطقية للفرد.
4. وسائل الإعلام (Communication Media): ويقصد بذلك أهمية دور وسائل الإعلام كالإذاعة والصحافة والتلفاز...الخ، في زيادة المحصول اللغوي للطفل.
5. عملية التعلم (Learning process ): ويقصد بذل أهمية عملية التعلم وما تتضمنه من قوانين التعزيز والاستعمال والإهمال التي تلعب دوراً مهماً في تعلم اللغة.
6. القدرة العقلية (Intecutal Ability): ويقصد بذلك أن أهمية القدرة العقلية (الذكاء) في النمو اللغوي للطفل، فالطفل الذي يتميز بذكاء عال يفوق الأطفال العاديين والمعوقين عقلياً في محصوله اللغوي، ويتميز باكتسابه اللغة في عمر زمني مبكر مقارنة مع الأطفال العاديين والمعوقين عقلياً، كما تظهر الكلمة الأولى لدى الطفل المعوق عقلياً في نهاية السنة الثالثة تقريباً، وهذا ما تؤكده الدراسات حول اهمية القدرة العقلية في النمو اللغوي.

تعريف اضطرابات الكلام واللغة:
• اضطراب الكلام: بأنها مشكلات يواجهها الطفل في الإنتاج الشفوي للغة سواء في النطق أو الطلاقة أو الصوت، والأطفال ذو الاضطراب الكلامي هم اولئك الذين يعيق كلامهم تواصلهم مع الآخرين.
• اضطراب اللغة: تعرف بأنها المشكلات التي يواجهها الطفل في رموز اللغة أو القواعد والتسلسلات اللازمة لربط هذه الرموز، بما يعنيه ذلك من مشكلات في فهم معاني الكلمات والجمل (لغة استقبالية) واستعمالها استعمالاً مناسباً ( لغة تعبيرية) وعلى هذا فإن اضطرابات اللغة يمكن أن تصيب شكل اللغة أو محتواها أو استعمالها في التواصل مع الآخرين.

أولاً: اضطرابات النطق:
إننا عندما نتحدث عن النطق فإننا نقصد بذلك قيام أعضاء النطق بعملها بالشكل المطلوب، وبالتالي انتاج كل صوت بشكل طبيعي، وإن أي خلل أو اضطراب في قيام أي عضو من أعضاء النطق، يجعلنا نقول: إن اضطراباً نطقياً قد نتج عن ذلك فما هو إذن اضطراب النطق:
يعرف الزراد (1990). اضطراب النطق بأنها تلك العملية التي يتم من خلالها التركيز على أي خلل يحدث في عملية وطريقة النطق، وطريقة لفظ الأصوات، وتشكيلها، أو إصدار الأصوات بشكل صحيح.
بينما تم تعريف اضطرابات النطق في الطبعة الرابعة من الدليل DSMIV (1994) الدليل التشخيصي الإحصائي للاضطرابات العقلية الصادر عن الجمعية الأمريكية للطب النفسي.
بأنه فشل في استخدام أصوات الكلام المتوقعة نمائياً والتي تكون مناسبة لعمر الفرد وذكائه ولهجته، ويتضح في إصدار صوتي ردئ أو تلفظ غير مناسب... ويتألف الاضطراب النطقي من أخطاء في: إصدار الصوت أو إبدال صوت مكان آخر، أو حذف أصوات مثل الحروف الساكنه التي تقع في آخر الكلمة، أو تشويه وتحريف لنطق الكلمة...ألخ. مما يعطي انطباعاً بأنه كلام طفولي.
أما جمال الخطيب ومنى الحديدي (1997). فقد عرفا اضطرابات النطق بأنها: أخطاء كلامية تنتج عن أخطاء في حركة الفك والشفاه واللسان أو عدم تسلسلها بشكل مناسب بحيث يحدث استبدال أو تشوه أو إضافة أو حذف وقد لا يكون لهذه الاضطرابات أسبابه العضوية الواضحة، وفي هذه الحالة تعزى اضطرابات النطق للحرمان البيئي والسلوك الطفولي، والمشكلات الانفعالية وبطء النمو.
وفي نفس السياق فقد عرف عبد العزيز الشخص (1999) اضطرابات النطق بأنه هو ذلك الاضطراب الذي يحدث نتيجة وجود أخطاء في إخراج أصوات حروف الكلام ومخارجها، وعدم تشكيلها بصورة واضحة وصحيحة، وتختلف درجات اضطرابات النطق من البسيطة إلى الحاد، حيث يخرج الكلام غير مفهوم نتيجة الحذف، والإبدال، والتشوية، والإضافة.
وهكذا يمكن تعريف اضطرابات النطق بأنها خلل في نطق الطفل لبعض الأصوات اللغوية يظهر في واحدة أو أكثر من الاضطرابات التالية: إبدال ( نطق صوت بدلاً من صوت آخر) أو حذف (نطق الكلمة ناقصة صوتاً أو أكثر) أو تحريف وتشويه ( نطق الصوت بصورة تشبيه الصوت الأصلي غير أنه لا يماثله تماماً) أو إضافة (وضع صوتاً زائداً إلى الكلمة).

أشكال اضطرابات النطق واللغة:
أولاً: اضطرابات النطق مايلي:
1. الإبدال Substitution:
ويحدث فيه استبدال الطفل نطق صوت بصوت آخر، كأن يستبدل الطفل نطق صوت (ر) بصوت (ل) فيقول مثلاً لجل بدل رجل، وملوحة بدلاً من مروحة... ويقع الإبدال مع أصوات أخرى مثل ابدال (ج) بصوت (د) فيقول الطفل دمل بدالً من جمل ودبنة بدلاً من جبنة ويستبدل أيضاً صوت (ك) بصوت (ت)، فيقول تتاب بدلاً من كتاب وسمته بدلاً من سمكة، ومن أبرز حالات الإبدال ما يسمى (بالثأثأة) Sigmatism وهي التي يبدل فيها الطفل صوت (س) بأصوات أخرى ومن أشكال الثأثأة:
- ابدال صوت (س) بصوت (ث) وهو ما يعرف Interdentalis, Sigmatism وترجع تلك الحالة التي بروز طرف اللسان خارج الفم بين الأسنان بدلاً من تراجعه إلى خلف الأسنان.
- إبدال صوت (س) بصوت (ش)، وهو ما يعرف باسم Lateral Sigmatism وترجع إلى انتشار تيار الهواء على جانبي اللسان بسبب عدم قدرة الطفل على التحكم في حركات لسانه أو بسبب تكوين هذا العضو.
- إبدال صوت (س) بصوت(ت) أو (د) وهو ما يطلق عليه Adentalis Sigmatism يحدث نتيجة ارتفاع اللسان إلى أعلى الثنايا العليا في منطقة أعلى من التي ينطق عندها صوت (س).
- نطق صوت (س) أنفية وهو ما يطلق عليه Nasal Sigmatism ويحدث نتيجة خروج الهواء من الأنف بدلاً من خروجه من الفم ويحدث ذلك نتيجة لوجود شق في سقف الحلق.
2. الحذف: Omission:
فيه يقوم الطفل بحذف صوت أو أكثر من الكلمة، وعادة ما يقع الحذف في الصوت الأخير من الكلمة مما يتسبب في عدم فهمها، إلا إذا استخدمت في جملة مفيدة، أو في محتوى لغوي معروف لدى السامع، وقد لا يقتصر الحذف على صوت، إنما قد يمتد لحذف مقطع من الكلمة فيقول الطفل (مام) بدلاً من (حمام) ويقول (مك) بدلاً من سمكة.
وقد يتم الحذف عند توالي صوتين ساكنين في أي موقع من الكلمة، دون أن تكون هناك قاعدة حذف ثابته ومحددة، أي أن الطفل قد يحدث الصوت الساكن الأول فيقول ( مرسة ) بدلاً من ( مدرسة).
وتسبب عملية الحذف صعوبة في كلام الطفل، ومعرفة الحاجة، أو الفكرة التي يريد التعبير عنها، مما يؤثر على الطفل، ويؤدي إلى ارتباكه وشعوره بعدم القدرة على توصيل أفكاره للآخرين.
وبصورة عامة يتصف الأطفال الذين يعانون من الحذف بمايلي:
1. إن كلامهم يتميز بعد النضج أو الكلام الطفلي Childish: وتشير نتائج الدراسات إلى أن الحذف يعد من اضطرابات النطق الحاد، سواء بالنسبة لفهم الكلام أو التشخيص وكلما زاد الحذف في كلام الطفل صعب فهمه.
2. غالباً يقل الحذف في كلام الطفل مع تقدم العمر، ومع ذلك فقد يظهر لدى الكبار ممن
3. يعانون من خلل في أجهزة النطق، أو اضطرابات في الجهاز العصبي، وكذلك الأطفال الذي يعانون من التوتر الشديد وأولئك الذين يتحدثون بسرعة كبيرة.
4. غالباً ما يميل الأطفال إلى حذف بعض أصوات الحروف بمعدل أكبر من الحروف الأخرى فضلاً عن أن الحذف يحدث غالباً في مواضع معينة من الكلمات، فقد يحذف الأطفال أصوات/ ج/، /ش/، /ف/، /ر/، إذا أتت في أول الكلمة أو أخرها بينما ينطقها إذا أتت في وسط الكلمة.
3. التحريف أو التشويه: Distortion:
وفيه ينطق الطفل الصوت بشكل يقربه من الصوت الأصلي، غير أنه لا يشبهه تماماً أي ينطق الطفل جميع الأصوات التي ينطقها الأشخاص العاديون، ولكن بصورة غير سليمة المخارج عند مقارنتها باللفظ السليم، حيث يبعد الصوت عن مكان النطق الصحيح، وتستخدم طريقة غير سليمة في عملية إخراج التيار الهوائي اللازم لانتاج ذلك الصوت. ويحدث التحريف نتيجة لعدة أسباب منها مايلي:
أ‌. تأخر الكلام عند الطفل حتى سن الرابعة.
ب‌. وجود كمية من اللعاب الزائد عن الكمية الطبيعية.
ت‌. أزدواجية اللغة لدى الصغار أو بسبب طغيان لهجة على أخرى.
ث‌. تشوه الأسنان بتساقط الاسنان الأمامية أو على جانبي الفك السفلي.
ج‌. قد ينتج عن مشكلة كلامية، كالسرعة الزائدة في الكلام مثلاً.
من نماذج التحريف في كلام الطفل:
روح – أوح ولد – ألد
كثير – تيل صحة – إحه
خلاص – هلاس قلم – ألم.
ولتوضيح هذا الاضطراب يمكن وضع اللسان خلف الأسنان الأمامية إلى أعلى دون أن يلمسها، ثم محاولة نطق بعض الكلمات التي تنطق أصوات مثل (س) و (ز) مثل: ساهر، زاهر، زايد، زياد، سهران، سالم، سامي
5. الإضافة: Addition:
فيه يصنف الطفل صوتاً زائداً إلى الكلمة، مما يجعل كلامه غير واضح وغير مفهوم ومثل هذه الحالات إذا استمرت مع الطفل أدت إلى صعوبة في النطق مثال ذلك: سمكة، مروحة، وغيرها، أو تكرار مقطع من كلمة أو أكثر مثل: واوا دادا.

محكات الحكم على اضطرابات النطق:
1. العمر الزمني:
تنمو الأصوات على شكل كلمات منطوقة بصورة عادية ضمن مدى معين من الأعمار المختلفة، لهذا هناك 13 صوتاً تبرز واضحة مابين الثالثة حتى الخامسة، في حين تظهر بقية الأصوات التي تتكون من 11 صوتاً بشكل صحيح ما بين السادسة حتى الثامنة، حيث تشكل تلك الأصوات مجتمعة صيغة الكلام التي لا يمكن للإنسان الصغير أن يتعلمها في آن واحد، وذلك لأسباب نمائية معروفة.
وعلى هذا الأساس هناك من يرى أن بعض الأصوات قد يتم اكتسابها مبكراً، وبعض الأصوات الأخرى بالإمكان اكتسابها وتعلمها بعد سن السابعة، غير أنه في الغالب يمكن فهم كلام معظم الأطفال بسهولة ما بين سن الثالثة والرابعة، ولذلك يعتمد نمو الأصوات وإخراجها على اعتبارات كثيرة، لعل من أهمها مرحلة النضج النمائي التي يمر بها الطفل والتي تغطي العديد من جوانب النمو المختلفة.
ولقد أجرى كل من (رو) ، (وملس) Roe & Milisen دراسة على الفي طفل من ولاية أنديانا الأمريكية، فوجد أن تعلم نطق الأصوات اللغوية يستمر حتى بعد قبول الطفل في المدرسة الابتدائية، هذا وتشير نتائج هذه الدراسة إلى أن تكرار الأخطاء النطقية يقل بشكل ملحوظ خلال الصفين الثالث والرابع، حتى في المدارس التي لا تطبق برامج وتدريبات خاصة بتصحيح النطق، وأظهرت النتائج أيضاً بأن التحسن الطبيعي على النطق يتزايد بشكل ملموس في الصفين الأول والثاني، بينما تتراجع القدرة على التغلب على الأخطاء النطقية في الصفين الثالث والرابع الابتدائيين.
وهكذا يمكن القول بأن الطفل قد يستمر بالنطق الخاطئ للأصوات الصامته حتى نهاية العام الثامن من عمر الطفل، وبنسبة عالية تتغلب على صعوبات النطق عند انتهاء الصف الاول و الثاني الأساسين وخاصة عند توفر البيئة التعليمية المناسبة في الفصول الدراسية، ويحدث هذا التحسن بدون أية برامج خاصة بمعالجة مشكلات النطق، أما إذا تعدت اضطرابات النطق لدى الطفل هذه المرحلة العمرية فإن هذا يعني أن الطفل بحاجة لعلاج اضطرابات النطق حتى لا تثبت وتصبح أكثر رسوخاً.

2. القدرة العقلية:
في بعض الحالات الخاصة التي يعاني فيها الأطفال من التخلف العقلي، أو التوحد، أو الشلل الدماغي، ينبغي تطبيق اختبار للذكاء لتحديد القدرات العقلية لهؤلاء الطلبة أو الأطفال.
إذ يتأثر نطق الطفل بمستوى القدرة العقلية التي يتمتع بها أفراد تلك الفئات، إذ أن هناك علاقة وثيقة بين مستوى النمو اللغوي والذكاء.
فالطفل العادي يبدأ نطق الأصوات بمحاكاة ما يسمعه من المحيطين به، بالألعاب الصوتية والمناغاة التي يقوم بها، وغالباً ما تكون هذه الأصوات في بادئ الأمر غير صحيحة، إلا أنه يحسن نطقه شيئاً فشيء حتى يستطيع النطق بشكل صحيح فيما بين الرابعة والسادسة أو السابعة من أعمارهم، بينما في مقابل ذلك نجد أن الطفل الذي يعاني من التخلف العقلي يتأخر في بداية النطق عن الطفل العادي، فالمهارات الكلامية لدى الأطفال المتخلفين عقليا تماثل تلك المهارات الموجودة لدى العاديين الذين يماثلونهم في العمر العقلي، وليس في العمر الزمني، بل أن بعض الحالات تظهر نمواً في تلك المهارات أقل من مستواه مما هو عليه لدى من يماثلونهم في العمر العقلي من الأفراد العاديين، هذا فضلاً عن أن اضطرابات النطق توجد بمعدل أكبر منها لدى الأطفال العاديين، وتزداد هذه الاضطرابات بزيادة درجة الإعاقة، أي أن العلاقة بين اضطرابات النطق ودرجة الإعاقة العقلية علاقة طردية.

3. إعاقة التواصل:
إن الطفل الذي لا يستطيع التعبير عن نفسه، وعما يدور بين الأخرين، أو التواصل معهم بسبب اضطراب نطقه، قد يؤدي به ذلك إلى الوقوع في العديد من المشكلات التي من بينها تجنب المستمعين له، أو تجاهله، أو الابتعاد عنه بسبب صعوبة التواصل والتفاعل معه، وعدم مقدرتهم على فهمه، ومن ثم استجابتهم له بصورة غير مناسبة، مما يؤدي إلى حدوث حالة من الارتباك بينهم وبينه، مما يترتب عليه اخفاق الطفل أو فشله في التواصل مع الأخرين وممارسة حياته الاجتماعية بشكل طبيعي.
ولا يقف الأمر عند هذا الحد، بل أن اخفاق الطفل في التواصل مع غيره يؤدي به إلى الوقوع في المشكلات النفسية نتيجة لما يعانيه من اضطراب في النطق ومنها: الخجل، والاحباط والأنطواء، والقلق الاجتماعي وغيرها من الأعراض الأخرى.
ويلاحظ على مثل هذا الطفل أيضاً صعوبة إحراز تقدم مشابه لما يحرزه زملاؤه في دراسته، مما قد يدفعه إلى تصرفات قد تكون غير سوية كالسلوك العدواني تجاه الآخرين، أو النشاط الزائد، وذلك لم يتعرض له من سخريه واستهزاء زملائه به.

ثانياً: اضطرابات الكلام:
لا شك أن الكلام هو من نعم الله على البشر ومن أهم وسائل التواصل بالآخرين ويستدعي كونه عدة توافقات عصبية دقيقة يشترك في أدائها الجهاز التنفسي لتوفير التيار الهوائي للنطق، وإخراج الأصوات بواسطة الحنجرة والحبال الصوتية والميكانزم السمعي للتميز بين الأصوات والمخ والجهاز العصبي السليم ونطق الحروف باستخدام اللسان والاسنان والشفاه وسقف الحلق الصلب والرخو والفك.
قال تعالى على لسان موسى عليه السلام: ( رب اشرح لي صدري ويسر لي أمري وأحلل عقدة من لساني يفقهوا قولي )طه: 24-28.
كذلك عني العرب بالعيوب اللغوية وأمراض الكلام فقد درسها الجاحظ في كتابه البيان والتبين.
وهذه الاضطرابات تتعلق بمجرى الكلام أو الحديث، ومحتواه، ومدلوله أو معناه وشكله، وسياقه، وترابطه مع الأفكار والأهداف، ومدى فهمه من الأخرين أسلوب الحديث، والألفاظ المستخدمة، وسرعة الكلام.
ويقصد بذلك تلك الاضرابات اللغوية المتعلقة بالكلام وما يرتبط بذلك من مظاهر تربط بطريقة تنظيم الكلام ومدته، وسرعته ونغمته وطلاقته، وتشمل اضطرابات الكلام المظاهر التالية:
* اللجلجة: Stuttering:
وتعني احتباس في الكلام يعقبه انفجار للكلمة بين شفتي الطفل مضطربةً بعد معاناة تتمثل في حركات ارتعاشية وتعتبر طبيعية من عمر 2-5 سنوات بعد ذلك تحتاج لبرنامج علاجي نفسي وكلامي.

أهم أشكال اللجلجة:
- تكرار الحرف أو الكلمة عدة مرات مثل كلمة (محمد) ينطقها (م م م م محمد).
- التو المفاجئ والطويل قبل نطق الحروف أو الكلمة ثم نطقها دفعة واحدة – توقف – محمد.
أسبابها:
تعود في الغالب لمرحلة الطفولة المبكرة، حيث يتأثر الطفل سلباً من الرعاية الزائدة أو الحرمان العاطفي، وتضارب أساليب التربية داخل الأسرة، أو الشقاء العائلي وكثرة المخاوف والسخرية التي تعرض لها الطفل.
التأتأة في الكلام: Stuttering:
وهي عدم الطلاقة في سيوله أو سلاسة الكلام بشكل يلفت النظر، مما يعيق التحدث مع الأخرين، والمتأتأ يكرر حرفاً أو مقطعاً بشكل لا إرادي مصحوباً باضطراب في التنفس، وحركات غريبة في اللسان، مما يسبب له الخجل والارتباك والعزلة، وهي لدى الذكور أكثر من الإناث.
ظاهرة السرعة الزائدة في الكلام (Cluttering):
حيث يزيد المتحدث من سرعته في نطق الكلمات، ويصاحب تلك الحالة مظاهر جسمية وانفعالية غير عادية مما يؤدي إلى صعوبة فهم المتحدث ومشكلات في الاتصال الاجتماعي.
ويكون العلاج بتنظيم عملية التفكير لدى المريض بعرض صورة امامه وعليه أن يراعي الترتيب المنطقي أثناء عرضه للحوادث الواردة فيها.
ظاهرة الوقوف أثناء الكلام (Blocking):
في هذه الحالة يقف المتحدث عن الكلام، بعد كلمة أو جملة ما لفترة غير عادية، مما يشعر السامع بأنه انتهى من كلامه، مع أنه ليس كذلك، وتؤدي اضطرابات الكلام أياً كانت إلى صعوبات في التعبير عن الذات تجاه الأخرين.
ضعف المحصول اللغوي:Delayed or ionhibited:
يدخل عامل الوراثة والقدرة على العقلية والسمعية وطبيعة العائلة وعامل الجنس دوارً في تأخر الكلام، فالبنات أكثر تقدماً في عملية الكلام بسبب طيلة الوقت التي تقضيه بجانب أمها أكثر من الذكور الذين ينصرفون إلى اللعب

ثالثاً: اضطرابات اللغة:
1- الحبسة (Aphasia):
تتأثر بعض مراكز اللغة في الدماغ نتيجة التعرض لحوادث أو انسداد في شرايين الدماغ مما يؤدي إلى ما يعرف بالحبسة وتتجلى بفقد القدرة على التعبير بالكلام أو الكتابة أو عدم القدرة على فهم معنى الكلمات المنطوق بها، أو عدم القدرة على إيجاد الأسماء ومراعاة القواعد النحوية ومن أنواعها:
‌أ. الأفازيا اللفظية: Verbal aphasia: ويكون المريض عاجزاً عن احضار الكلمات قولاً وكتابة
‌ب. أفازيا اسمية :Naminal Aphasia: حيث يجد المريض نفسه عاجزاً عن فهم أسماء الأشياء أو معنى الكلمات ( كل كلمة على حده).
‌ج. أفازيا نحوية: Syntactic Aphasia: حيث يكون المريض غير قادر على تركيب الجملة مع مراعاة القواعد.
‌د. أفازيا معنوية: syntactic Aphasia: حيث يكون المريض غير قادر على تركيب الجملة مع مراعاة القواعد.

2. عسر الكلام أبراكسيا: Apraxia:
عدم التحكم بانتاج الكلام بشكل إرادي نتيجة عدم القدرة على التنسيق بين الجهاز العصبي والعضلي، مثل حالات الشلل الدماغي حيث يجب التدريب المستمر لأعضاء النطق التي يصعب تحريكها بالاستعانة بأخصائي التنفس والعلاج الطبيعي وفي بعض الحالات يصعب الوصول إلى نتيجة مرضية فنلجأ إلى وسائل التواصل البديلة كالإرشادات.
3. تأخر ظهور اللغة (Language Delay):
وفي هذه الحالة لا تظهر الكلمة الأولى للطفل في العمر الطبيعي لظهورها، وهو السنة الأولى من عمر الطفل بل قد تتأخر ظهور الكلمة إلى عمر الثانية أو أكثر، ويترتب على ذلك مشكلات في الاتصال الاجتماعي مع الآخرين وفي المحصول اللغوي للطفل وفي القراءة والكتابة فيما بعد.
4. صعوبة الكتابة (Dysgraphia):
في هذه الحالة لا يستطيع الطفل أن يكتب بشكل صحيح المادة المطلوبة كتابتها، والمتوقع كتابتهما ممن هم في عمره الزمني، فهو يكتب في مستوى يقل كثيراً عما يتوقع منه .
5. صعوبة فهم الكلمات أو الجمل:Echolailia agnosia:
يقصد بذلك صعوبة فهم المعنى للكلمة أو الجملة المسموعة وفي هذه الحالة يكرر الفرد استعمال الكلمة أو الجملة دون فهمها.
6. صعوبة القراءة (Dyslexia):
في هذه الحالة لا يستطيع الطفل أن يقرأ بشكل صحيح المادة المكتوبة، والمتوقع قراءتها ممن هم في عمره الزمني، فهو يقرأ في مستوى يقل كثيراً عما يتوقع منه.
7. صعوبة تركيب الجملة ( Language Deficit):
ويقصد بذلك صعوبة تركيب كلمات الجملة من حيث قواعد اللغة ومعناها لتعطي المعنى الصحيح وفي هذه الحالة يعاني الطفل من صعوبة وضع الكلمة المناسبة في المكان المناسب.

رابعاً: اضطرابات الصوت: Voice disorders
ويتضمن أي اضطراب يختص بعلو الصوت أو انخفاضه أو خشونته بشكل غير سوي ويمكن أن نحدد هذه المشكلات بمايلي:
1. اضطراب الإيقاع الصوتي ويشمل:
أ‌. ارتفاع الصوت.
ب‌. انخفاض الصوت:
ويقصد بذلك ارتفاع الصوت أو انخفاضه بالنسبة للسلم الموسيقي وطبقة الصوت، والصوت المرتفع أكثر من اللازم هو صوت شديد ومزعج للسامعين، والصوت الطبيعي يكون على درجة كافية من الارتفاع أو الشدة من أجل تحقيق التواصل المطلوب فالارتفاع الشديد يؤدي إلى صوت غير واضح.
ج-الفواصل في الطبقة الصوتية: ويقصد بذلك التغيرات الغير طبيعية في طبقة الصوت، والانتقال السريع الغير مضبوط من طبقة لأخرى، مثل الانتقال من الصوت الخشن إلى الناعم أو الرفيع مما يؤدي إلى عدم وضوح اللحن والصوت.
د‌- الصوت المرتعش أو المهتز:
وهو صوت غير متناسق من حيث الارتفاع أو الانخفاض أو الطبقة الصوتية يكون سريعاً ومتوتراً (الخوف) (الانفعال).
هـ. الصوت الرتيب:
وهو الصوت الذي يأخذ شكل واحد، وإيقاع واحد، ووتيرة واحدة، دون القدرة على التغيير في الإيقاع، والشدة والنغمة واللحن، مما يجعل هذا الصوت يبدو شاذاً، وغريباً.
2. مشكلات في شدة الصوت كالصوت الخشن والصوت الطفلي وبحه الصوت
أ‌. الصوت الخشن أو الغليظ: Harshness:
ويكون هذا الصوت من النوع المرتفع في الشدة المنخفض في الطبقة الصوتية ويكون هذا الصوت مصحوباً بالتوترات، والإجهاد، وقد تكون خشونة الصوت لدى الصغار بسبب الصراخ العالي أو تقليدهم لأصوات الأخرين، كذلك الأفراد ذوي المزاج العدواني غالباً ما يجهدون حبالهم الصوتية في صراخهم وحديثهم.
ب‌. بحة الصوت:
إن الصوت المبحوح هو صوت منخفض الطبقة الموسيقية وقد يصاحبه شيء من خشونة الصوت أو غالباً ما يكون البح بسبب الاستخدام السيء للصوت ويكون أيضاً بسبب تقارب أشرعة الحبال الصوتية، وهذه البحة يصاحبها صعوبة في التنفس، وقد تؤدي أيضاً حالات التهاب الحنجرة، أو النزلة البردية، والتهاب اللوزتين والإجهاد الكلامي إلى بحة الصوت.
ح‌. الصوت الطفلي:
وهو الصوت الذي نسمعه لدى الراشدين أو الكبار ويشبه في طبقته الصوتية طبقة صوت الأطفال الصغار ( الصوت الرفيع الحاد) بحيث يشعر السامع بأن هذا الصوت شاذاً لا يتناسب مع عمر وجنس ومرحلة نمو الفرد المتكلم.
3. الخمخمة: Rhinolalia:
وهو خروج الكلام من الأنف بصورة مشوهة غير مألوفة فينطق حرف (م) (ب) أو (د) بسبب وجود فجوة في سقف الحلق أو سد فتحات الأنف.

أسباب اضطرابات اللغة والنطق:
هناك مجموعة من العوامل التي تكون مسؤولة عن هذه االاضطرابات منها ما يرجع إلى عوامل عضوية أو نفسية او اجتماعية، وفيما يلي فكرة عن هذه الاسباب:
• أسباب عضوية:
كاختلال الجهاز العصبي المركزي، واضطراب الأعصاب المتحكمة في الكلام، أو الضعف العقلي ونقص في خلايا الدماغ، أو الإصابة بالصمم، أو بسبب عيوب الجهاز الكلامي، مثل اختلال أربطة اللسان وسد فتحات الأنف أو تضخم اللوزتين وشق الحلق، وكذلك سلامة الأجهزة النطقية مثل الحنجرة ومزمار الحلق والفكين والأنف والشفتين والأسنان واللسان تعد شرطاً من شروط سلامة الفرد من الاضطرابات اللغوية.
• أسباب نفسية:
وهي الأسباب الغالبة على معظم عيوب النطق كالقلق والصراع والصدمات والمخاوف وعدم الشعور بالأمن، أن الكثير من المصابين ببعض الاضطرابات الكلامية يتكلمون بصورة طبيعية عندما يكونوا بأمان وبمفردهم ولكن يعود الاضطراب لهم إذا وجدوا في مواقف محرجة ومع آخرين يمثلون السلطة عليهم، وهناك أسباب أخرى مثل:

- أنماط الكلام الأخرىين التي يتعرض لها الطفل أثناء تعلم الكلام.
- تمييز أحد الأخوة عن الاخر.
- المشاحنات المستمره بين الأبوين.
- وجود الطفل في بيئة تتعدد فيها اللهجات واللغات.
- إجبار طفل المسر على الكتابة باليد اليمنى
- إبجار الأباء أولادهم على النطق.
- استجابة لحاجات الطفل دون كلام.
- الاسرة كثيرة السكوت قليلة الكلام.
• الاسباب المرتبطة بإعاقات أخرى:
ويقصد بذلك أن الاضطرابات اللغوية ظاهرة مميزة لدى الأفراد ذوي الاعاقات العقلية والسمعية، والانفعالية، وصعوبات التعلم، لكن الكثير من مظاهر اضطرابات اللغة والنطق مرتبطة بتلك الإعاقات، فعلى سبيل المثال تمثل ظاهرة تأخر ظهور الكلام أو اللغة، وظاهرة التوقف أثناء الكلام، أو الكلام بصوت غير مسموع من المظاهر المميزة للإعاقة العقلية في حين أن قلة المحصول اللغوي وظاهرة غياب اللغة من المظاهر المميزة للاعاقة السمعية، في حين تمثل مظاهر صعوبة الفهم للغة وتذكرها وظاهرة التأتأة والسرعة الزائدة في الكلام والإضافة والإبدال والحذف والتشويه وتغيرات الوجه والجسم غير العادية أثناء الكلام من المظاهر المميزة لحالات الإعاقة الانفعالية وصعوبة القراءة والكتابة وسوء تركيب الجملة مظاهر لصعوبات التعلم.

نسبة الاضطرابات اللغوية:
تختلف نسبة الأفراد ذوي الاضطرابات اللغوية تبعاً لاختلاف الدراسات أو الأبحاث التي أجريت حول موضوع نسبة الاضطرابات اللغوية من حيث أنواعها واسبابها، وعلى سبيل المثال يقدر مكتب التربية في الولايات المتحدة الأمريكية نسبة الاطفال ذوي الاضطرابات اللغوية في المجتمع الأمريكي بحوالي 3.5% وتظهر دراسة أجراها بيركن (Perkin, 1971) أن نسبة أطفال المدارس ممن يعانون من اضطرابات في الكلام تتراوح من 1% إلى 3%.
أما كيرك (Kirk, 1972) فيشير إلى دراسات إجريت أيضاً في الولايات المتحدة الأمريكية حول نسبة الأفراد ذوي المشكلات اللغوية ومن تلك الدراسات الدراسة التي أجرتها الجمعية الأمريكية للسمع والكلام على عينة من 40 مليون تقريباً من الأفراد الذين تتراوح أعمارهم ما بين 5 سنوات إلى 21 سنة وقد أشارت نتائج هذه الدراسة إلى ما نسبته 5% من تلك العينة تعاني من مظهر ما من مظاهر اضطرابات النطق باللغة وإن أعلى نسبة من نسب الاضطرابات اللغوية هي نسبة اضطرابات النطق باللغة، وأن أعلى نسبة من نسب الاضطرابات اللغوية، هي نسبة ذوي المشكلات النطقية إذا بلغت تلك النسبة 3%.
وفي الأردن تذكر أبو غزالة (1986)، دراسة مسحية لحالات اضطرابات اللغة في مدارس وزارة التربية والتعليم في الأردن للعام الدراسي 83/82 وأشارت تلك الدراسة المسحية إلى أن عدد حالات الطلبة ذوي المشكلات النطقية هو 94 حالة.

قياس وتشخيص الاضطرابات اللغوية.
تتلخص عملية قياس وتشخيص الاضطرابات اللغوية في أربعة مراحل أساسية متكاملة هي:
المرحلة الأولى: مرحلة التعرف المبدئي (Screening ) على الأطفال ذوي المشكلات اللغوية:
وفي هذه المرحلة يلاحظ الآباء والأمهات والمعلمون والمعلمات، مظاهر النمو اللغوي وخاصة مدى استقبال الطفل للغة، وزمن ظهورها والتعبير بواسطتها والمظاهر غير العادية للنمو اللغوي مثل التأتأة أو السرعة الزائدة في الكلام أو قلة المحصول اللغوي...الخ. وفي هذه المرحلة يحول الأباء والأمهات أو المعلمون والمعلمات الطفل الذي يعاني من مشكلات لغوية إلى الأخصائيين في قياس وتشخيص الاضطرابات للغوية.
المرحلة الثانية: مرحلة الاختبار الطبي الفسيولوجي (Physical Diagnosis) للأطفال ذوي المشكلات اللغوية:
وفي هذه المرحلة وبعد تحويل الأطفال ذوي المشكلات اللغوية أو الذي يشك بأنهم يعانون من اضطرابات لغوية إلى الأطباء ذوي الاختصاص في موضوعات الأنف والأذن والحنجرة (Ent Doctors) وذلك من أجل الفحص الطبي الفسيولوجي وذلك لمعرفة مدى سلامة الأجزاء الجسمية ذات العلاقة بالنطق واللغة، كالأذن والأنف، أو الحبال الصوتية واللسان والحنجرة.
المرحلة الثالثة: مرحلة اختبار القدرات الأخرى ذات العلاقة للأطفال ذوي المشكلات اللغوية Diagnosis of Abilities:
وفي هذه المرحلة وبعد التأكد من خلو الأطفال ذوي المشكلات اللغوية من الاضطرابات العضوية يتم تحويل هؤلاء الأطفال إلى ذوي الاختصاص أو الإعاقة العقلية والسمعية والشلل الدماغي، وصعوبات التعلم، وذلك للتأكد من سلامة أو إصابة الطفل بإحدى الإعاقات التي تم ذكرها سابقاً وذلك بسبب العلاقة المتبادلة بين الاضطرابات اللغوية للطفل ونوع الإعاقة التي يعاني منها ويستخدم ذو الاختصاص في هذه الحالات الاختبارات المناسبة في تشخيص كل الإعاقات العقلية أو السمعية أو الشلل الدماغي أو صعوبات التعلم.
المرحلة الرابعة: مرحلة تشخيص مظاهر الاضطرابات اللغوية للأطفال ذوي المشكلات اللغوية Diagnosis Language Disorders :
في هذه المرحلة وعلى ضوء نتائج المرحلة السابقة يحدد الاخصائي في قياس وتشخيص الاضطرابات اللغوية، مظاهر الاضطرابات اللغوية التي يعاني منها الطفل ومن الاختبارات المعروفة في هذا المجال:
1. اختبار الينوي للقدرات السيكولغوية والذي يتكون من 12 اختبارً فرعياً يصلح هذا الاختبار للفئات العمرية من سن 2-10.
2. اختبار مايكل بست لصعوبات التعلم ويتكون هذا الاختبار من 24 فقرة موزعة على خمسة أبعاد.
3. مقياس المهارات اللغوية للمعوقين عقلياً ويتألف هذا المقياس من 81 فقرة موزعة على خمسة أبعاد هي:
أ‌. الاستعداد اللغوي المبكر .
ب‌. التقليد اللغوي المبكر.
ت‌. المفاهيم اللغوية الأولية.
وهناك اختبارات أخرى يذكرها كومبتن ( Compton, 1980) هي:
1. اختبارات دوتيرويت للاستعداد للتعلم (Dtroit tests Learning Aptitude).
2. اختبار سلنقرلاند المبدئي على الأطفال ذوي الصعوبات اللغوية المحددة (Slingrland screening Tests 1974).
3. اختبار فشر – لوقمان للكافية النطقية (The Fisher – Logemann Test).
4. اختبار الاستيعاب السمعي للغة: (test for Audditory Comprehenstion of Language by Corrowe 1977).
5. مقياس كومبتن الصوتي: (Copton- Hutton Phonological Assessment

الخصائص السلوكية والتعليمية للأطفال المضطربين لغوياً:
هل تؤثر الاضطرابات اللغوية على الخصائص السلوكية للأطفال أو الأفراد ذوي المشكلات اللغوية؟ وما هي مظاهر ذلك التأثير؟ فهل هو في مظاهر قدراتهم العقلية؟ أو التحصيلية أو الاجتماعية؟ تتضح الاجابات عن مثل تلك الأسئلة إذا ما استعرضنا الخصائص السلوكية لذوي الاضطرابات اللغوية والتي يمكن تصنيفها في الخصائص التالية:
1. الخصائص العقلية: ويقصد بالخصائص العقلية أداء المفحوص على اختبارات الذكاء المعروفة مثل مقياس ستانفورد بينية أو كسلر ويشير هلهان وزميله كوفمان (Halahan- Kauffman, 1983) إلى تدني أدىء ذوي الاضطرابات اللغوية على مقياس القدرة العقلية، مقارنة مع العاديين المتناظرين في العمر الزمني، وفي الوقت الذي يصعب فيه تعميم مثل ذلك الاستنتاج، إلا أن ارتباط الاضطرابات اللغوية بمظاهر الإعاقة العقلية أو السمعية والانعفالية أو صعوبات التعلم أو الشلل الدماغي يجعل ذلك الاستنتاج صحيحاً إلى حد ما وعلى ذلك فليس من المستغرب أيضاً أن نلاحظ تدني أداء ذوي الاضطرابات اللغوية على اختبارات التحصيل الأكاديمي مقارنة مع العاديين خاصة إذا أضفنا أثر العوامل النفسية والاجتماعية في تدني التحصيل.
2. الخصائص الأكاديمية: ليس غريباً من أن الطلبة الذين يعانون من اضطرابات لغوية ذوي تحصيل ضعيف في اللغة من جهة وفي كل مظاهر التعلم التي تعتمد على اللغة من جهة أخرى، فمفرداتهم بشكل عام محدودة وسطحية وتعكس القصور الذي يعانونه في القدرة على تطوير المفاهيم وبخاصة المفاهيم المجردة، ثم أنهم يميلون إلى التعامل مع الأشياء أو الحوادث كأمور كلية أكثر من ميلهم إلى التعامل مع التفاصيل ربما ليخففوا من عبء استعمال اللغة والكلام.
كذلك الأطفال ذوو الاضطرابات اللغوية يميلون إلى توسيع استخدام الكلمات في سياقات غير ملائمة كأن يستخدموا عصير لكل ما يمكن سكبه في كأس الحليب والماء وغير ذلك أو أن يعنونوا كل ما يطير بكلمة طير أو أن يستخدموا كلمة (بس) (قط) لتدل على كل ما يمشي على أربعة.
ومعلوم أن الأطفال ذوي الاضطرابات في اكتساب اللغة يواجهون صعوبات متفاوته في تعلم القراءة، وبخاصة عندما نزداد مطاليب الكفاءة في القراءة بعد الصف الثالث، حيث لا بد من القدرة على قراءة الموضوعات العلمية الأخرى كالمواد الاجتماعية والعلمية، كما أن جنوح بعض المواد إلى استخدام المجردات والرموز تضاعف من صعوبات ذوي الاضطرابات اللغوية. 3. الخصائص الانفعالية والاجتماعية والنفسية:
غالباً ما تكون القدرات الأدائية في اختبارات الذكاء ضمن الحدود السوية لذوي الاضطرابات اللغوية، غير أن الملاحظ أن معظم القدرات العقلية تعتمد على اللغة أكثر من اعتمادها على العمل والأداء، ولهذا يلاحظ بأن درجات ذوي الاضطرابات اللغوية في اختبارات الذكاء تقل في الجانب اللغوي عنه في الجانب الأدائي، لأنهم بشكل عام يميلون نحو الحرفية في تفكيرهم ويفتقدون إلى المرونة اللازمة للمجاملة والتفاعل الاجتماعي الفعال، ثم أن اختلالات اللغة التعبيرية مثلاً تحد من تطوير أشكال رفيعة من اللعب الجماعي، لهذا فإنه لا يتاح لذوي الاضطرابات اللغوية إلا القليل من فرص تعلم المهارات الاجتماعية والتواصلية ومهارات اللعب الجماعي.
ويلاحظ أن بعض مشكلات النطق البسيطة تستمر مع الطفل وتلازمه في الرشد ولكنها لا تسبب له مشكلة في حين أن التخلف اللغوي الحاد يمكن أن يؤثر على كل مظهر وظيفي منه للفرد وقد يكون له آثار عميقة على التطور التربوي والانفعالي والاجتماعي.
وقد وجد بالدراسة أننا إذا نظرنا إلى الأطفال ذوي الاضطرابات التواصلية كمجموعة فإننا نجد كثير من المشكلات النفسية تشيع بينهم، ومن ذلك أن عدم النضج، وقصر فترة الانتباه، والقابلية للتهيج، ونوبات الغضب والسلوك الانعزالي ومشكلات السلوك من مميزات الأطفال ذوي الاضطرابات الكلامية واللغوية لملازمة سلوكهم الشخصي (Bakeretal, 1980).
ومع طول الفترة التي يعاني منها الأطفال اللجلجة تزداد إمكانية معاناة المشكلات الانعفالية، فهم يعون بشيء من الألم ردود فعل المستمع نحوهم سواءً ابتعاداً عنهم أو اشفاقاً عليهم، مما يشعرهم بالارتباك والذنب والإحباط والغضب كما ينزع الملجلج إلى الشعور باليأس ويفقد الثقة بنفسه ويصبح قلقاً ومضطرباً وأكثر ميلاً للانسحاب من الفعاليات الاجتماعية.
ويلاحظ بأن كثيراً من هؤلاء يستجيبون إلى مصاعبهم بالعدوان أو انكار معاناتهم للمشكلة بينما يميل آخرون إلى تجنب المواقف التي قد يطلب منهم فيها الكلام، فمثل هذه المواقف تشحن انفعالاتهم ولهذا فإن عدم معالجة اللجلجة واستمرارها ملازمة للفرد يمكن أن تؤدي إلى مشكلات تربوية واجتماعية خطيرة لهذه الفئة، مثل الشعور بالرفض من الآخرين أو الأنطواء والانسحاب من المواقف الاجتماعية أو الإحباط والشعور بالفشل أو الشعور بالنقص، أو بالذنب أو العدوانية نحو الذات أو نحو الآخرين أو العمل على حماية أنفسهم بطريقة مبالغ فيها أو ما يعبر عنه باسم الحماية الزائدة.

علاج اضطرابات النطق والكلام:
من الضروري أن يبدأ الأطفال الذين يعانون من اضطرابات في اللغة في الحصول على العلاج قبل أن يصل إلى سن المدرسة، لأن اكتساب اللغة عادة يتم على مدار الخمس سنوات الأولى من عمر الطفل، وفيما يلي عرض لبعض طرق العلاج المستخدمة مع هذه الفئة:
1. العلاج الجسمي:
من خلال التأكد من أن المريض لا يعاني من اسباب عضوية خصوصاً من النواحي التكوينية والجسمية في الجهاز العصبي المركزي وأجهزة السمع لأن السمع هو أول خطوات اكتساب اللغة، فإذا كان ضعف السمع هو المسبب فيمكن التغلب عليه بواسطة سماعات الاذن أو زراعة الوقعة لبعض الحالات التي تعاني من ضعف شديد.
2. العلاج النفسي:
ويكون بتقليل التوتر النفسي وتنمية شخصيته ووضع حد لخجله ومعرفة الصعوبات التي يعاني منه والعمل على معالجتها.
3. العلاج الكلامي:
وهو علاج مكمل للعلاج النفسي ويجب أن يلازمه، وهو أسلوب للتدريب على النطق الصحيح عبر جلسات متعددة عن طريق اخصائي النطق ويتم تدريب المريض عن طريق
- تقليد الكلمات.
- الاسترخاء الكلامي: حيث يجعل المريض في حالة استرخاء بدني وعقلي ثم يبد في قراءة قطعة ببطء شديد مع إطالة في كل مقطع يقرأه مثل: بندور ب...ن...دو...رة.
- تمرينات الكلام الإيقاعي: أي ربط كل مقطع من الكلمة بواحد من الإيقاعات الآتية: تصفيف بالأيدي أو الضرب باحدى القدمين على الأرض.
- تدريب جهاز النطق والسمع عن طريق استخدام المسجلات الصوتية وتقوية أجهزة النطق والتدريبات الرياضية أيضاً.
- تظليل الكلمات: حيث يقوم المريض بترديد ما يقوم به الأخصائي أخصائي النطق واللغة من كلمات وجهاً لوجه، في نفس الوقت وبفارق جزء من الثانية.
- أسلوب النمذجة: حيث يأخذ الطفل المريض زمام المبادأة ويقوم المعالج بتكرار الجمل الناقصة التي يتلفظ بها الطفل ويضيف إليها الكلمات الناقصة بهدف توسيع ما يتلفظ به الطفل.
4. العلاج البيئي:
إن إدماج الطفل في نشاطات اجتماعية ورياضية وفنية وجعله يلعب مع أطفال أخرين، حتى يتدرب على الأخذ والعطاء ويتاح له فرصة التفاعل وتنمية الشخصية.
إن عيوب النطق التي تستمر عند الطفل حتى مع توفير الاستثارة الإضافية والدلالات التي يقدمها المعالج يصعب في العادة تدريب الطفل على تصحيحها وتظل سمة ترافق كلامه وبعض الأشخاص لا بد لهم من الاستعانة بوسائل أخرى بما لديهم من قدرات للتواصل مع الآخرين ومعرفة ما يجري، كإشارات اليد، تعبيرات وإيماءات الوجه التي تقوم مقام الكلمات.

تمارين مساعدة للنطق والكلام:
- التحكم بحركات اللسان أمام المرأة.
- فتح الفم وإخراج اللسان بشكل مروس للخارج دون لمس الأسنان أو الشفاه ثم إعادته للداخل ببطء.
- فتح الفم قدر المستطاع وجعل اللسان يلامس الشفه العليا ثم السفلى ببط ثم بسرعة.
- فتح الفم وجعل اللسان يلامس الأسنان في الفك الأعلى ثم الأسفل ببطء وبسرعة.
- فتح الفم وجعل اللسان يقوم بعملية نقله من اليمين إلى الشمال وبالعكس.
- فتح الفم وجعل اللسان يقوم بعملية دائرية حول الشفاه.
- إغلاق الفم وتحريك اللسان بشكل دائري.
- فتح الفم وإدخال اللسان إلى الوراء وجعله يلامس أخر الفك الأعلى.
- نفخ شمعة أو نفخ فقاعات صابون أو ريشة.
- ترقيص لهب شمعة عن بعد لأطول فترة ممكنه.
- جذب الهواء للداخل كتمرين التثاؤب لرفع سقف الحلق.
- تدريب الطفل على التنفس السليم لإخراج الصوت ( شم وردة من الأنف ثم النفخ من الفم)
- تدريب الشفاه بنطق الحروف الصوتية، أ،و،ي، أأ، وو،ي ي.
- تقسيم الكلمات إلى مقاطع مثل: تلفون، ت،ل،فو،ن.

البرامج التربوية لذوي الاضطرابات اللغوية.
يقصد بالبرامج التربوية لذوي الاضطرابات اللغوية طرق تنظيم برامج الأطفال ذوي الاضطرابات اللغوية وتحديد نوعية تلك البرامج، وتعتبر مراكز الإقامة للأطفال ذوي الاضطرابات اللغوية من أقدم تلك البرامج، حيث تقدم لهؤلاء الأطفال أو الذين يمثلون في الغالب ( الإعاقة العقلية، أو السمعية، أو الانفعالية، أو الشلل الدماغي، أو صعوبات التعلم) برامج صحية واجتماعية وتربوية في المراكز نفسها، يلي ذلك مراكز التربية الخاصة النهارية، ثم الصفوف الخاصة الملحقة بالمدرسة العادية، ثم دمج الطلبة ذوي المشكلات اللغوية في الصفوف العادية وهو يمثل الاتجاه التربوي الحديث في تنظيم الأطفال ذوي المشكلات اللغوية فلا بد وإن تتضمن تلك البرامج المهارات الأساسية التالية في تعلمهم وهي:
1. مهارة تعليم الأطفال ذوي الاضطرابات اللغوية من قبل اخصائيين في تعليم اللغة واضطراباتها ويطلق على هذا الاخصائي مصطلح (Speech Therapist) وتبدو مهمة هذا الاخصائي في قياس وتشخيص مظاهر اضطرابات النطق واللغة ومن ثم وضع البرامج التربوية الفردية المناسبة لكل منهم.
2. مهارة تعليم الأطفال ذوي الاضطرابات اللغوية وفق مبادئ تعديل السلوك. والممثلة في أساليب التعزيز الإيجابي أو السلبي أو العقاب أو تشكيل السلوك أو التقليد...الخ.
3. مهارة اختيار الموضوعات المناسبة للتحدث عنها، وتبدو مهمة معلم الأطفال ذوي الاضطرابات اللغوية والعمل مع الطلبة أنفسهم على اختيار الموضوعات المحببة أو المشوقة للأطفال، إذ يعمل ذلك على تشجيع الأطفال على الحديث عن تلك الموضوعات، كما يعمل على تخفيف التوتر الانفعالي لدى الطلبة ذوي الاضطرابات الانفعالية.
4. استماع معلم هذه الفئة لطلبته ولحديثهم دون أن تبدو عليه مظاهر صعوبة في قبول هؤلاء الطلبة ذوي المشكلات اللغوية، وخاصة الأطفال ذوي المشكلات اللغوية المتمثلة في التأتأة أو السرعة الزائدة في الكلام.
5. مهارة تشجيع وتعزيز الأطفال ذوي المشكلات اللغوية، وخاصة مشكلات التأتأة أو السرعة الزائدة.
6. مهارة تقليد نطق الكلمات أو الجمل بطريقة صحيحة، ويقصد بذلك تشجيع الأطفال ذوي المشكلات اللغوية على تقليد ونمذجة الأخرين ذوي النطق الصحيح وتعزيزها لديهم، وخاصة إذا ما استخدم اسلوب تحليل المهارات والمصحوب بالتعزيزات الإيجابية.
7. مراعاة مهارات تعليم الأطفال ذوي الإعاقة العقلية، والسمعية، والانفعالية والشلل الدماغي، وصعوبات التعلم، والذين يظهرون اضطرابات لغوية وخاصة فيما يتعلق باستخدام الإيحاءات أو الاشارات، أو النماذج الكلامية.
ويذكر الروسان (2000)، في كتابه مقدمة في الاضطرابات اللغوية، عدداً من أساليب تدريس المهارات اللغوية وفق أسلوب الخطة التعليمية الفردية حيث تضمن ذ1لك الكتاب (120) هدفاً تعليمياً في المهارات اللغوية ومنها الأمثلة التالية:

الهدف العام: أن يحرك الطفل راسه نحو الأصوات الصادرة بالقرب منه
الهدف السلوكي: أن يحرك الطفل رأسه نحو مصدر الصوت القريب منه
المواد اللازمة: جرس
الأسلوب التعليمي: 1. قرع الجرس بالقرب من اذني الطفل أثناء لعبة دون رؤيته لك
2. لاحظ تحريك الطفل لرأسه نحو مصدر الصوت مرة نحو اليسار ثم مرة نحو اليمين.
3. شجع الطفل لفظياً عند استجابته لمصدر الصوت
معيار النجاح في أداء الهدف السلوكي: يعتبر الطفل ناجحاً في أدائه للهدف السلوكي عندما يحرك رأسه نحو مصدر الأصوات القريبة منه

الهدف العام: أن يفهم الطفل اللغة الاستقبالية
الهدف السلوكي: أن يشير الطفل إلى أجزاء جسمية مثل الأنف، العين، الفم.
المواد اللازمة: لعبة وصورة
الأسلوب التعليمي: 1. أشر إلى كل جزء من أجزاء جسم الطفل مثل: الأنف، العين، الفم.
2. أطلب من الطفل أن يشير إلى تلك الأجزاء مشجعاً إياه على الإشارة إليها.
3. كرر الإجراء الثاني بأن يشير إلى كل جزء من أجزاء جسمه بشكل منفرد.
4. عزز أداء الطفل عندما يشير بشكل صحيح إلى تلك الأجزاء.
معيار النجاح في أداء الهدف السلوكي: يعتبر الطفل ناجحاً في أدائه للهدف السلوكي عندما يشير بشكل صحيح إلى أجزاء جسمه المطلوبة.

مجال الصعوبة: اللغة التعبيرية
الهدف العام: أن يعرف الطفل استعمال الأشياء اليومية
الهدف السلوكي: أن يعبر الطفل عن استعمالات الأشياء اليومية
المواد اللازمة: كرة، حذاء، فنجان، ملعقة، قلم، غطاء للرأس
الأسلوب التعليمي: 1. أسأل الطفل أسئلة مثل: لماذا نستعمل الفنجان؟ لماذا نلبس غطاء الرأس؟
2. كرر الإجراءات السابقة مع أشياء أخرى.
3. ضع الأشياء السابقة في حقيبة ودع الطفل يخرج واحدة منها في كل مرة ويخبرك عن استعمالاتها، (أخبره أنت إذا لم يستطع ذلك).
4. نوع النشاطات المقدمة للطفل بإضافة أشياء أخرى.
5. شجع الطفل وعززه على الإجابات الصحيحة.
معيار النجاح في أداء الهدف السلوكي: يعتبر الطفل ناجحاًُ في أدائه للهدف عندما يعبر عن استعمال خمسة أشياء مألوفة لديه.

مجال الصعوبة: اللغة التعبيرية
الهدف العام: أن يسمي الطفل الألوان الأساسية
الهدف السلوكي: أن يسمي الأطفال الألوان الأساسية التالية: أحمر، أزرق، أخضر، أصفر
المواد اللازمة: مكعبات ملونة ذات ألوان أحمر، أزرق، أصفر، أخضر
الأسلوب التعليمي: 1. ضع المكعبات الملونة في الحقيبة ثم أخرج المكعب الأحمر سائلاً الطفل ما هو لون هذا المكعب.
2. كرر الإجراء السابق مع الألوان الباقية.
3. عزز الطفل على تسميته للألوان السابقة.
4. أطلب من الطفل أن يسمي ألوان أشياء أخرى في غرفة الصف لها نفس الألوان السابقة.
المعيار في أداء الهدف السلوكي: يعتبر الطفل ناجحاً في أدائه للهدف السلوكي عندما يسمى أربعة ألوان بشكل صحيح.

ملاحظات:
ارشادات ونصائح للأسرة والمدرسة
1-يجب مراعاة النمو اللغوي لدى الطفل الصغير وتدريبه على الكلام واتاحة الفرصة لمخاطبته، وفهمه مما يؤدي الى النمو اللغوي لدى الطفل، كما يجب احاطة الطفل بالرعاية والعطف والحنان، وبخاصة من قبل الام، وتوفير الرعاية الصحية والنفسية بشكل صحيح.
كذلك يفضل على الاباء مطالعة بعض الكتب التثقيفية حول اللغة وتطورها ومشكلاتها، وهذا ما يساعدهم على فهم الغة لغة طفلهم وطبيعة نموها، والعوامل المؤثرة في هذا النمو.
2- اذا لاحظ الاباء وجود قصور لغوي واضح لدى طفلهم بالمقاربة مع الاطفال الاخرين ينبغي مراجعة الاخصائيين من الاطباء، والاخصائي النفسي او اخصائي التخاطب، حتى يتم مواجهة المشكلة ان وجدت بشكل مبكر وبالاعتماد على مبادىء علميه سليمة، ويجب عدم قلق الاباء كثيرا لان مسألة اللغة هي مسألة وقت، ونضج عصبي، وعضوي، وفكري، كما انها مسألة رعاية وتربية.
3- يجب الانتباه الى اللغة العامية واللغة الفصحى عند تعلم الطفل الكلام، وكذلك تعويد الطفل على عدم استخدام الالفاظ الشاذة والبذيئة ..... ويفضل عدم تصحيح الاخطاء النحويه، لان هذا يعتبر امر طبيعي يجب المرور عليه، وبالتالي فإن محاولة التصحيح القسرية تؤدي الى مضاعفات او اضطربات لغويه، ويجب عدم الاسراف في ذلك.
4-اذا كان للطفل اخوة صغار يجب عدم مقارنة لغة الطفل او نطقه مع اخواته الذين يتكلمون افضل منه، لان ذلك يترك اثرا سيئا لدى الطفل ويزيد من مشكلته .
5- ان وضع الطفل في عمر سنتين في دور الحضانه لاسباب مثل عمل الام او المرض او غير ذلك مقبول ولكن يفضل لو يكون الطفل مع اسرته وبين اخواته الذين يكبرونه سنا .
6-تشجيع الطفل على الكلام، التحدث، والتعبير بطلاقه، والاستماع الصحيح والقراءة، وسماع القصص، والحكايات الشيقة، والاهتمام بالصور والرسومات ويفضل عدم اجابة الطفل عن اسئلته بالاشارات والحركات الايمائيه او المبالغة في ذلك.
7- على الام التي تعمل خارج المنزل ايجاد الفرصة المناسبة لتعويض الطفل عما ينقصه من رعاية، وعطف وحوار لغوي ان من واجبات الام ليس تقديم الطعام والشراب فقط بل لا بد من الجانب الروحي والمعنوي الهام لدى الطفل .
8-ضرورة الانتباه إلى خطورة ازدواجية اللغة، حيث يعيش الطفل خلال فترة نموه اللغوي داخل وسط يتدرب فيه أو يستمع إلى لغتين مختلفتين تماما، وهذا ما يعيق نمو اللغة ويحدث اضطرابات في شخصية الطفل (أثر المربيات الاجنبيات)
9-اما بالنسبة للطفل الأعسر فيجب اعتبار ان هذه الحالة عاديه غير مرضية ويفضل عدم الإلحاح أو الضغط على الطفل للكتابة بيده اليمنى لان ذلك قد يترتب عليه فشل دراسي واضطراب شخصي، وتأتأة في الكلام.
10- ان الطفل قد يكتسب اضطرابات النطق والكلام من أبويه واخواته، والمقربون اليه، اذا تضمن كلامهم اخطاء في النطق والكلام، فالطفل سرعان ما يكتسب او يقلد، ويتعود على نطق هذه الكلمات، او الاحرف، او الجمل بشكل خاطئ,ويظهر اثر ذلك في مراحل النمو الاولى للطفل وفي الاضطرابات الوظيفية للنطق والكلام .
11- على المدرس ان يكون واعيا بالفاظ ونطق تلاميذه الصحيحة، والخاطئة، والفروق الفردية اللغوية بين تلاميذه,والاستفادة من تدريبات القراءة الجهرية، او تدريبات الهجاء في تحسين بعض الاضطرابات المتعلقة بذلك كما يمكنه التعاون مع الاسرة في تحسين مستوى النطق والكلام لدى الطفل.
12-من الضروري على المؤسسات التربوية اجراء فحوص واختبارات دورية تتناول مستوى السمع والابصار، والقدرة العقلية العامة، والقدرة اللغوية والسلوك العام لدى الاطفال والتلاميذ.
13-العمل على وقاية الطفل من الامراض والاضطرابات ومتابعة مراحل نموه اللغوي بشكل صحيح.

نصائح مهمة للاسرة التي لديها طفل مصاب باضطرابات في اللغة والنطق
1-الانصات بصبر الى حديث الطفل وعدم الالتفات للطريقة التي يتحدث بها .
2-تكرار الكلمات التي يقولها الطفل بشكل صحيح .
3-نطق الكلمة التي يقولها الطفل بشكل صحيح بزيادة كلمة او اثنتين معها.
4-التركيز على الاصوات التي ينطقها الطفل بصورة صحيحة ولكنه يحذفها او يستبدلها.
مثال:قال(تمك )هنا نقول له، س,س,س,سمك.
5-تعليم الطفل بهدوء الكلمات التي يحتاجها للتعبير عن شعوره .
6-سؤال الطفل اسئلة متعددة الخيارات مثلا :تريد حليبا,ام عصيرا
7-النظر اليه بصورة طبيعية اثناء كلامه.
8-الغناء للطفل اناشيد جديدة .
9-قراءة القران الكريم يوميا للطفل .
10-اضافة مفردات على الجمل التي يقولها الطفل اذا قال عصير نقول (كرم يرد عصيرا)
11-مدح الطفل عندما يعبر عن شعوره وافكاره.
12-اطلب من الطفل ان يوصل المهام لفظيا مثال( بابا يريد شايا )
13-اطلب من الطفل ان يقلد اصوات بعض الحيوانات ( قطة ، كلب ، حصان ،بقرة )
14-عدم انتقاد الطفل واجباره على تغير طريقة كلامه وتصحيح اخطاءه باستمرار .
15-اغلاق التلفاز او الراديو عند تناول الطعام مع الاسرة.
16-توفير جو منزلي مفعم بالهدوء والامان .
17-اختيار كلمات التدريب من البيئة المحلية (كرسي، شجرة، محمد,بيت)
18-قراءة الطفل للكتاب او قصص تناسب مستوى التعليمي.

The study of voice, swallowing and airway disorders is referred to a laryngology, a subspecialty of otolaryngology (the study of ear, nose, and throat disorders).  Indiana University now has a clinic that specializes in laryngologic disorders such as hoarseness, swallowing problems, airway narrowing (subglottic or tracheal stenosis), and chronic cough. Dr. Stacey Halum, the director of the new clinic, works together with board-certified speech language pathologists to meet patients' laryngology needs. Dr. Halum is a board-certified otolaryngologist who completed a laryngology fellowship at Wake Forest University, and she is currently the only fellowship-trained laryngologist practicing in the state of Indiana.
The following material is a general educational tool for patients and their families.  It is not recommended that patients self-diagnose or self-treat on the basis of this information, and we recommend any person with these disorders be evaluated by a laryngologist.
How does normal speech production occur?Under normal conditions, air from the lungs is sent through the vocal folds (voice box) to generate the "power" for voice.  The vocal folds come together as the rush of air passes between them, and this produces rapid vibration of the vocal folds resulting in the sound of the human voice.  The vibration of the vocal folds is what causes the pitch or "frequency" of the human voice.  Adult female voices are much higher pitched than males because female vocal folds generally vibrate faster (180-240 times per second) than those of males (80-130 times per second).  After the air passes through the vocal folds to make voice, it continues up through the throat (pharynx) which adds additional quality to the sound of the voice, and then it comes out of the mouth.  The mouth is the site where the voice is formed into words- this act of forming sounds and words is called articulation. Hoarseness, or dysphonia, is an abnormality of the voice.  This is generally caused by problems with the vocal folds themselves, but occasionally lung or airway problems can also contribute to hoarseness, since strong airflow is needed to produce voice.

How are articulation disorders different than voice disorders?Disorders that impair the normal pronunciation of words are generally considered articulation disorders, while disorders that cause a hoarse or strained voice are called voice disorders.  Examples of two common articulation disorders are Velopharyngeal Insufficiency (VPI) and Stuttering.
Velopharyngeal Insufficiency (VPI)While forming sounds or words is generally related to the mouth (lips, teeth, tongue), sounds are also highly affected by the airflow through the nose.  The soft palate (back portion of the roof of the mouth) is generally responsible for allowing or blocking airflow escape through the nose during sound production. For example, air is allowed to travel up through the nose to create "mmm" and "nnn" sounds, but airflow to the nose needs to be blocked to create sounds like "pa" and "ba".  In a condition called "velopharyngeal insufficiency" (VPI), the palate does not bock airflow escape out of the nose, making pronunciation of certain sounds difficult.  With severe VPI, patients may even regurgitate liquids out of the nose upon attempted swallowing.  There are a variety of treatments for VPI including prosthetics, surgery, and speech therapy.
StutteringStuttering involves repetitive sounds that interrupt the normal flow of speech.  It is a disorder of articulation, rather than actual voicing.  Speech therapy is currently the best treatment available for stuttering, but certain centers have ongoing trials for medical interventions.
What are examples of voice disorders?Voice disorders can have a wide number of etiologies.  Some disorders affect the fluency of speech, which prevent people from having continuity of their words or sentences.  Examples of disorders that affect fluency include spasmodic dysphonia (SD) and muscle tension dysphonia (MTD).  SD and MTD can both present similarly, with a strained-sounding voice with episodic voice breaks.  These breaks interrupt words, making speech sound irregular.  Examples of voice disorders that make the voice sound hoarse include vocal fold paresis or paralysis, benign or malignant (cancerous) vocal fold growths, and laryngopharyngeal reflux (LPR)
Spasmodic Dysphonia and TremorSpasmodic dysphonia (SD) is a voice disorder that affects voice fluency.   Patients generally have interrupted, strained sounding speech.  Patients also complain that speaking requires extra effort, and they feel fatigued after prolonged voice use/speaking. SD is a neurologic disorder, and therefore does not tend to respond well to speech therapy alone.  While SD is a neurologic disorder and not a psychiatric/psychological problem, it does tend to be aggrevated by stressors and stressful situations, such as talking on the phone or in front of groups.  SD has several subtypes. 
Adductor (Ad) SDThe most common SD subtype is Adductor (Ad) SD.  These patients notice breaks in the voice primarily when saying vowels.  On examination, their vocal folds are 'squeezing' together inappropriately in the middle of words, interrupting the normal flow of speech.  This subtype of spasmodic dysphonia responds extremely well to botulinum toxin (Botox ) injection, with over 90% of patients attaining improved fluency.  These injections are given every three to six months, depending on the patient response. 
Abductor (Ab) SDAbductor (Ab) SD is another subtype of SD that, like Ad SD, is characterized by voice breaks.  Unlike Ad SD, however, Ab SD involves breaks with our shortly after consonants rather than vowels.  On examination, the vocal folds pull apart suddenly in the middle of words, producing breathy sounding breaks.  Ab SD is much more rare than Ad SD.  While botulinum toxin injections are also the standard of care for Ab SD, it tends to have a much poorer response than Ad SD. 
Tremor
Many patients with SD also have a coexistent voice tremor.  Tremor can be present with or without SD.  Unlike SD, which produces irregular voice breaks, tremor tends to produce more regularly spaced breaks, that can occur with both consonants and vowels.  On examination the vocal folds are continuously 'twitching' open and closed, and this tremulous movement causes interruptions in normal voicing.  Unfortunately, tremor tends to be extremely difficult to treat, with a poor response to standard botulinum toxin injections, although IU Clinic for Voice, Swallowing, and Airway disorders, we are now doing variations in the standard botulinum toxin injections, and have had very encouraging results.
Muscle Tension Dysphonia (MTD)Muscle tension dysphonia (MTD) is a voice disorder caused by excessive 'squeezing' or tension of the voice box with speaking.  Often vocal fold weakness (partial or complete paralysis) will result in MTD, because straining or squeezing is necessary to bring the vocal folds for adequate closure and voice.  These patients will often complain of fatigue and discomfort with prolonged voice use, and extra-effort required to speak.  Most patients with MTD alone will respond very well to speech therapy.  If patients have MTD because of underlying vocal fold weakness (see vocal fold paresis or paralysis), then they may need a procedure done to help them get adequate vocal fold closure.  This usually involves an injection or placement of an implant to add bulk to the vocal fold for better laryngeal (voice box) closure.
Vocal Fold Paresis or Paralysis
If one of the vocal folds does not move normally, as in the case of partial (paresis) or complete vocal fold paralysis, the vocal folds will not be able to come together, and air will simply pass through them with little to no vibration.  This results in a very breathy sounding voice, and patients often feel very short of breath with talking because the persistent gap between the vocal folds is like a "leaky valve," allowing rapid air escape.  If the vocal fold is partially paralyzed, it is called a vocal fold paresis.  If it is completely immobile, it is called paralysis.  The causes of vocal fold paresis or paralysis can vary widely, with endotracheal intubation, surgery, tumors, and trauma being some of the most common known causes.  In many cases the cause is unknown.  The procedures to correct this involve insertion or injection of material that pushes the paralyzed vocal fold to the midline, so it can meet the normal vocal fold, and the vocal folds will again be able to vibrate and produce voice.  If vocal fold injection is performed, it can be done in the clinic or in the operating room (see figure).  If an implant is inserted, this procedure is called a medialization laryngoplasty, and it is done in the operating room, using local anesthesia and sedation.

Benign Vocal Fold GrowthsBenign growths on the vocal folds most commonly result in a hoarse voice, which may range from rough to breathy.  Patients often feel fatigued with prolonged voice use, and require extra-effort to speak.  There are many different types of benign vocal fold growths, with some being self-limited, and primarily localized to the vocal folds.  Examples of these include vocal fold granulomas, nodules, cysts, and polyps (see pictures below).  Other growths can extend well beyond the voice box and produce airway obstruction and other problems.  An example of this is recurrent respiratory papillomatosis (RRP) (see pictures below).  When diagnosing vocal fold growths, the top priority needs to exclude malignancy or cancer.   This is generally done with a biopsy, either in clinic or in the operating room.  Benign vocal fold growths are generally treated with surgical excision.  While this can be done in the operating room, new technology is now allowing us to treat many of these growths in clinic (see Pulsed Dye Laser).  Patients tolerate this extremely well, and do not need to miss time of work or have the risks of general anesthesia, as would be needed with traditional surgery.

Malignant (Cancerous) Vocal Fold GrowthsThe  most common malignant vocal fold growth is called squamous cell carcinoma.  Presenting symptoms often include hoarseness and increased effort with speaking, with throat pain, ear pain, swallowing problems, and weight loss often being present as the cancer progresses.  Patients often, but not always, have a history of heavy smoking and/or alcohol use.   The diagnosis of cancer is made by obtaining a biopsy (tissue sample) of the growth.  While this traditionally was done in the operating room, new technologies now permit us to do this in clinic with many patients.  Treatment of squamous cell carcinoma of the vocal folds generally involves radiation or surgery when the cancer is limited to the true vocal folds alone.  Surgery, radiation and/or chemotherapy in various combinations may be necessary when the cancer is more advanced.

ARE THERE OPTIONS TO TRADITIONAL SURGERY FOR VOICE DISORDERS?
At Indiana University, we are rapidly advancing treatment options for our patients with voice and swallowing disorders.  For example, we have the latest technology including transnasal esophagoscopy (TNE) and the pulsed dye laser (PDL) that allow us biopsy and treat many vocal fold growths in the clinic, avoiding traditional surgery.   Thus, our patients can drive themselves home the same day and avoid taking day(s) off work, as would be required with traditional surgery. They also avoid the risks of general anesthesia, with excellent outcomes and virtually no complications.

LASER SURGERY IN A CLINIC SETTING
At Indiana University, many benign vocal fold growths can now be treated with a special laser surgery in a clinic setting.  This allows our patients to drive themselves home the same day and avoid taking day(s) off work, as would be required with traditional surgery. They also avoid the risks of general anesthesia, with excellent outcomes and virtually no complications.  The laser used at Indiana University is called the pulsed dye laser (PDL).  After the patients nose and throat are well numbed, the laser is passed through a special transnasal endoscope to just above the vocal fold growth.  Multiple pulses of laser energy are applied to the growth and the procedure is then completed, usually taking less than 10 minutes of actual laser time.  Patients generally have little to no pain, and can go back to work and resume normal activities immediately after the procedure.  The PDL has also been used to successfully treat papillomas, granulomas, polyps, and dysplasia of the vocal folds. The PDL is an ideal laser for clinic use because it does not penetrate deeply (only approximately 2mm) and does not significantly damage normal epithelium, so it produces much less scarring than other lasers, such as the CO2 laser.  It is also selectively absorbed by blood vessels, so there are no problems with bleeding.  In fact, it can be used after office-based biopsies to control bleeding, if necessary.  Finally, it is highly effective for treating many benign vocal fold growths, with small growths often requiring just one treatment.
TransNasal Esophagoscopy (TNE)TNE is a new technique that allows us to examine the esophagus in the office.  The TNE is a very thin endoscope that can be passed easily along the floor of the nose, and then advanced down the esophagus for a complete examination.  Because so many patients have acid reflux disease and because the incidence of esophageal adenocarconima (linked to acid reflux) is rapidly rising, clinic based esophageal screening tools like TNE are especially important.  The alternative to TNE is esophagoscopy done under deep sedation or general anesthesia.  In both cases the sedation or anesthesia required to perform the procedure generally requires that the patient lose a day from work. With TNE, only topical anesthesia is required so that patient may return to work shortly after the procedure is completed.  They also avoid the risks of general anesthesia.  The TNE scope can also be used for other procedures such as obtaining biopsies (tissue samples) of growths involving the throat (pharynx), voice box (larynx), or esophagus

Types of Dyslexia
Dyslexia is a learning disability which hinders a persons ability to spell or read. There are many different types of dyslexia. Dyslexia is diagnosed by a doctor such as a neuropsychologist, a developmental pediatrician, neurologist or educational psychologist.

Some symptoms of dyslexia include difficulty learning letters, distinguising word sounds, and confusion reading sentences in the correct order and direction.

Neglect Dyslexia
Neglect dyslexia is a condition where a person neglects to read one side of a word. This is most common in longer words, or compound words. For example when reading the word "stairway" the reader may only see "way" or "stair".

Spelling Dyslexia
Spelling Dyslexia is when the reader has problems reading all types of words. At times, as person with spelling dyslexia, may not be able to identify certain letters. Those with spelling dyslexia read very slow, and will read each letter at a time.

Semantic Dyslexia

Semantic Dyslexia is when a person has trouble understanding a word because they have distorted the spelling or meaning of the it. When reading a word they may think of a synonym, or antonym of the word. For example, a person with semantic dyslexia may confuse the word "jump" with "jumped".

Dyslexia Without Dysgraphia
Dyslexia without Dysgraphia is a form of dyslexia that may never be diagnosed in some patients because it typically does not affect a persons oral language or writing. Dyslexia with Dysgraphia is when a person has problems with reading but not with writing. Some with this form of dyslexia will also have trouble doing math because they have to read numbers which can be confusing.

Dyslexia With Dysgraphia
Dyslexia with Dysgraphia is commonly referred to as "Deep Dyslexia" because it is very severe. A person with this form of dyslexia may have problems writing letters and words, pronouncing words, and grasping the meaning of some words. This form of dyslexia can effect all aspects of someone's life, so it is important that someone with this form of dyslexia to receive additional help.

Trauma Dyslexia
This form of dyslexia occurs after a person has had a traumatic injury to trauma to the brain. Usually, for students who are of school age, this form of dyslexia is not diagnosed because they will receive a note of Traumatic Brain Injury (TBI).

Treating Dyslexia
It is crucial to diagnose dyslexia at an early age. The sooner it is diagnosed, the sooner it can be treated. For many school age children, it is best to sit down with the school and create and IEP (individual education program). An IEP will give the child special care and will help them to cope with their learning disability.


One of the first steps to treating dyslexia are to learn your child's weaknesses. Some children have trouble reading, while others have problems with speaking, math or writing. Some children may even have a combination of these challenges. Once you are aware of their areas of weakness you will know the areas to concentrate on most. A good place to start treating dyslexia is letters. Try using flash cards to review letters and sounds with your child. Then continue by working on phonics. Another suggestion is to hire a tutor who can work with you child. There are also many programs or games that your child can play which will help to treat their dyslexia.

Hearing Loss in Children
a risk factor for speech and language impairment
Hearing loss in children is a frequent contributing factor in speech and language impairment. A hearing loss can cause a speech or language disorder in some cases, or it can worsen an already existing speech or language disorder. For this reason, a hearing test is usually a part of a full speech-language evaluation.
If you think about it, it makes sense. To be capable of good language output, you have to have adequate input, and for most languages (sign languages being the obvious exception), the input has to travel through the ears to get to the brain. Hearing loss in children results in limited access to the speech sounds, vocabulary, and grammatical structures of their language; if their hearing loss goes undiagnosed and untreated during the first few years of life, it can have a lasting negative effect on their speech and language development.
What do I know, anyway?
In this section, as in the rest of my site, I am speaking out of my training and experience as a speech-language pathologist. As such, I have some basic training in audiology, but not to the extent that an audiologist would have. In my capacity as a speech-language pathologist, I do not perform full audiological evaluations or make recommendations regarding hearing aids or other amplification devices. I do perform hearing screenings, which are a simple pass-fail measure; if a child fails a screening, I refer the parents to an audiologist for further testing. If a child is diagnosed with a hearing loss, a speech-language pathologist often provides audiological rehabilitation to strengthen listening and auditory processing skills.
I am also speaking here as the father of a child with a unilateral hearing loss. My daughter was diagnosed with a moderately severe mixed hearing loss in the left ear at about age three. I'll talk more about her on my page about unilateral hearing loss.
All hearing losses are not created equal
The first thing to remember about hearing loss in children is that it can come in many different forms.
Before I get into that, though, I want to offer a few definitions:

  • The decibel scale is what we use to measure the loudness (a.k.a. intensity, amplitude) of sounds. The higher the number of decibels (dB), the louder the sound. The decibel scale is a bit tricky in that it is logarithmic instead of linear. Every increase of 10 dB is ten times louder (20dB is 10 times louder than 10 dB; 30 dB is 10 times louder than 20 dB, and 100 times louder than 10 dB, and so on). Also, keep in mind that zero decibels (0dB) is not a complete absence of sound; it is the quietest level at which the average healthy, young ear can hear sound. Some people can hear sounds at -5dB or even -10dB. Although most of us cannot hear sounds quieter than that, complete absence of sound can only exist in a vacuum, where there is no material for sound waves to travel through.
  • Hearing threshold is the quietest level at which a person can hear a sound. Thresholds between 0dB and 20dB are usually considered to be in the normal range. Lower thresholds indicate better hearing; people with thresholds above 20 dB are considered to have a hearing loss. A person may have a different threshold in the left ear than in the right. It's also common for thresholds to vary according to the frequency (pitch) of a sound. Hunters (especially those who don't wear hearing protection) sometimes have elevated thresholds for high frequencies corresponding to the those produced by gun blasts, but normal thresholds for lower-frequency sounds.
  • Frequency is the way we measure the pitch of a sound--how high or low it is. We measure frequency in cycles per second (CPS), also called Hertz (Hz). Sound is caused by vibrations, and the faster something vibrates (i.e., more cycles per second), the higher the sound will be. A typical adult speaking voice ranges from 100 Hz to 220 Hz on average; male voices tend to be at the lower end of that range, and female voices are usually toward the upper end. However, as we speak, we also produce higher frequencies, called harmonics or overtones, and it is these frequencies that change as we produce speech sounds. We need to be able to hear frequencies up to around 3000 Hz to be able to hear vowel sounds clearly, and to hear the difference between /s/ and /sh/ we need to hear up to between 7000 and 8000 Hz. Some types of hearing loss affect our ability to hear all frequencies equally, while others will affect some frequencies more than others. For example, noise-induced hearing loss often affects the higher frequencies more than the lower ones.

Degrees of hearing loss in children
Hearing losses range from mild to profound. A person with a mild hearing loss (hearing thresholds at 20-40 dB) may experience occasional difficulty hearing whispered words or very quiet sounds, or hearing what people are saying in settings with a lot of background noise. A lot of people with mild hearing loss are unaware that they even have a hearing loss. However, even this level of hearing loss in children can affect speech and language development, as well as success in school.
People with a moderate hearing loss (hearing thresholds at 40-70 dB) have difficulty hearing a normal conversation in a quiet room. Unstressed syllables, and words at the ends of sentences where the voice trails off, will be especially troublesome. School children with moderate hearing loss can benefit from a combination of amplification (hearing aids, FM system) and compensatory strategies (sitting close to the teacher, being positioned where they can see the teacher's face clearly).
Many professionals include a category called moderately severe hearing loss, which is typically the upper end of what others call "moderate": hearing thresholds between 55 and 70 dB. This is a helpful category to have, since there is actually a huge difference between 40 dB and 70 dB. Remember that the decibel scale is logarithmic, so a sound at 70 dB is 1,000 times louder than a 40 dB sound. People whose hearing is in this range often cannot hear people's voices at normal conversational levels, although they may be able to hear you if you raise your voice, especially if there is no background noise. Children with moderately severe hearing loss have a hard time learning effectively in a classroom setting unless they are using amplification and compensatory strategies.
A person with a severe hearing loss (hearing thresholds at 70-90 dB) cannot hear conversations held at normal levels. Only a loud voice close to the ear is audible. Severe hearing loss in children is associated with high risk for speech and language deficits. This risk can be reduced through the use of amplification or sign language in addition to compensatory strategies. There are people with this level of hearing loss who consider themselves deaf and use sign language as their primary mode of communication, and there are others who consider themselves hard-of-hearing and rely on amplification and oral communication.
A person with a profound hearing loss (hearing thresholds above 90 dB) is unable to hear any but the loudest noises, if that. Ninety decibels is about the level of a gas lawn mower. Even with hearing aids, most people with this level of hearing loss cannot hear spoken conversation. Children with profound hearing loss often do not benefit from hearing aids. Some parents of children with profound hearing loss elect to enroll their kids in special schools or programs for the deaf, while others choose cochlear implants for their children.
Different types of hearing loss in children
Conductive hearing loss. Plug your ears with your fingers. Are they plugged? Congratulations, you just gave yourself a conductive hearing loss. Fortunately, it's temporary (take your fingers out now--and go wash them before you touch the keyboard again!). A conductive hearing loss occurs due to blockage or structural damage in the ear canal or middle ear. All frequencies are affected equally, so this type of hearing loss can be easily corrected with amplification (hearing aids). In some cases, surgery can repair the damage or remove the blockage, restoring normal hearing. There is no such thing as a good hearing loss, but if you must have one, this is the kind to hope for.
Sensorineural hearing loss results from damage to the inner ear or the auditory nerve, which takes sound impulses from the inner ear to the brain. This type of hearing loss is irreversable and cannot be corrected with surgery. Also, the sensitivity to a given frequency depends on the site of the damage. In other words, you may be able to hear lower frequencies reasonably well, but have trouble with higher frequencies. When this happens, it is difficult, or even impossible, to distinguish among sounds like /s/, /sh/, and /f/. Correction for sensorineural hearing loss usually involves a digital hearing aid, which can be programmed to amplify the specific frequencies the wearer needs.
Mixed hearing loss occurs when there are elements of both conductive and sensorineural factors. Generally, there is damage to the middle ear and the inner ear, as a result of injury, illness, or congenital deformities. In some cases, surgery can correct the conductive issues, but the sensorineural component is irreversible.
In the case of a central auditory processing disorder (CAPD), the outer and inner ear and the auditory nerve are intact, but the brain has difficulty processing the sounds it receives. Areas of particular difficulty for children with CAPD include: listening in the presence of background noise; understanding and remembering directions, lists, and sequences; distinguishing between sounds in words (e.g., hearing bee when someone says key); maintaining attention to listening tasks; and higher-level listening tasks like drawing conclusions, making inferences, or interpreting math "story problems". Only an audiologist can diagnose CAPD, but treatment is usually done by a speech-language pathologist. This is a higher-level processing disorder, so a child with CAPD will be able to pass the kind of hearing screen the public schools provide (the beeps are easier to process than speech). For this reason, plus the fact that a lot of parents and teachers do not really know about CAPD, the approximately 5% of school-aged children who have it are often mis-diagnosed as having attention deficit disorder (ADD). In some very unfortunate cases, they are simply labeled "lazy" or "uncooperative", or accused of "selective hearing".
A bilateral hearing loss affects both ears, but it is possible that the degree or type of hearing loss may not be the same in both ears. For example, one ear may have a moderate conductive loss while the other may have a severe sensorineural loss. This is known as an asymmetrical hearing loss; a symmetrical loss is one in which the degree and type are the same in both ears.
A unilateral hearing loss, as the name suggests, occurs in just one ear; the other ear has normal hearing. A lot of people make the mistake of assuming that a unilateral hearing loss is "no big deal" because the person "can hear just fine" with the unaffected ear. Research has shown, however, that unilateral hearing loss in children is associated with increased risk for speech and language impairments and for difficulty in school. An additional factor with unilateral hearing loss is that it affects a person's ability to localize sound (tell which direction the sound is coming from).

Causes and prevention of hearing loss in children
Causes of hearing loss in children include a variety of genetic and environmental factors. About 50% of hearing losses are due to genetic factors. Some of these are recessive, so it is possible for a child of two parents with normal hearing to be born with a genetic hearing loss. Some environmental factors can cause hearing loss in children before birth (for example, the mother contracts rubella or cytomegalovirus during pregnancy), while others, like otitis media (middle ear infections), ototoxic medications, or noise exposure may cause hearing loss in children after birth.
Because so many cases of hearing loss are caused by environmental factors, education about hearing loss prevention can have a powerful effect on reducing the prevalence of hearing loss in children. Advances in medical diagnosis and treatment have reduced the incidence of illnesses that can cause hearing loss; at the same time, public education efforts have raised awareness of the dangers of noise exposure and the importance of hearing protection in noisy environments.
A fluctuating hearing loss may be caused by recurring bouts of otitis media. Fluid builds up in the middle ear and causes a conductive hearing loss. When the infection goes away, the fluid is re-absorbed (or drains out through a ruptured eardrum) and hearing returns to normal; however, the risk is always there that a viral infection could spread to the inner ear and result in a permanent hearing loss.
A temporary hearing loss may also result from an ear infection. Another possible cause is a build-up of ear wax, or cerumen, in the ear canal. Some people produce more cerumen than average and their ears become blocked as a result, unless they are proactive about managing the build-up. Ear wax buildup rarely causes hearing loss in children, however. Far more common is self-inflicted (or parent-inflicted) blockage of ear wax through the use of cotton swabs (Q-tips) to "clean" the ears. What many people do not realize is that swabs only remove a small amount of ear wax, and actually push most of it farther into the canal, where it hardens and eventually blocks the canal. Always consult a doctor before attempting to remove ear wax from your own ear or from your child's.
Diagnosing hearing loss in children
Until not too long ago, hearing loss in children tended to go undetected until a parent or other family member (usually the mother) noticed that something did not seem right. In the case of a severe hearing loss or deafness, the child might have been between one and two years in age when the parents noticed an absence of response to sound, such as turning toward a parent's voice, waking or startling at loud noises, and so on. Because children with less severe hearing loss, or a loss in just one ear, do respond to sound, these cases were often discovered even later, sometimes not until the child was in school. This is unfortunate, because the first years of a child's life are so important for speech and language development. Hearing losses that go undetected during this time can cause serious setbacks to speech, language, literacy, social interaction, and academic success.
Today, many industrialized nations require universal newborn screening for all babies born in hospitals. Newborns who fail the initial screening are re-screened, and if they fail the second screen, they are referred to an audiologist for a more thorough examination.
If the only kind of hearing test you have ever experienced is the one where you wear a headset and raise your hand when you hear a 'beep', you may be wondering how on earth one would test a newborn's hearing. The answer is that there are several passive tests to check how different parts of the auditory system are working. One type of test measures otoacoustic emissions (OAE), which are 'echoes' produced by the inner ear in response to sound. An earplug with both a speaker and a microphone are placed in the ear canal; the speaker emits a sound, and the microphone detects the echo. Another test detects auditory brainstem responses (ABR), or brainwaves produced in response to sound. Electrodes placed on the scalp detect activity in the brainstem following presentation of a tone through an earplug. For both of these tests, it is unnecessary for the child to raise a hand or indicate in any other way whether he heard the sound. In fact, the quieter and less active the child is, the better, and for ABR testing, it's best for the child to be asleep during the procedure.
It is important for parents, teachers, and others who work with children to be aware of the symptoms of hearing loss in children. There are two main reasons for this. First, I've never heard of a child going to his parent or teacher and saying, "You know, I think I may have a hearing loss. Maybe I should get tested." Children with a hearing loss are often unaware that anything is wrong with their hearing. They may notice that the grown-ups and other kids around them mumble a lot and then get upset with them when they don't understand, but it does not occur to them that this is due to their ears not working properly. Reason number two is that, when children miss something you say, they do not generally cup a hand behind their ear and say, "Eh? Speak up, sonny, I can't hear you!" In fact, if you ask them directly whether they heard you, children (with or without hearing disorders) will often answer "yes", because this is what they think you want to hear. The signs and symptoms of hearing loss in children are much more subtle, and adults who do not have hearing loss on their mind may easily mistake them for something else, like attention deficit disorder, a learning disability, autism, or just plain willfulness.
Symptoms of hearing loss in children will vary in their obviousness depending on the type and severity of the hearing loss. Common warning signs include:

  • not responding to a parent's voice at normal speaking level
  • absent startle response to loud sounds (with some types of hearing loss, however, loud sounds are amplified, even painful, and the child does startle.
  • inability to localize the source (tell where a sound is coming from)
  • difficulty following instructions or requests, despite appearing to listen attentively
  • reduced or absent responses to environmental noises (car horns, approaching footsteps, sirens outside, telephone ringing, doors opening or closing etc.)
  • consistently setting the volume at an unusually high level when watching television or listening to the radio.
  • enjoyment of rhythmic clapping games while appearing uninterested in music
  • immature speech-language development
  • social withdrawal
  • fatigue, frustration, and acting out


Treating hearing loss in children
Some types of conductive hearing loss can be corrected with surgery, or by removal of impacted ear wax. However, most hearing losses are not curable. Treatment usually focuses on some combination of the following elements:

Hearing loss in children is a huge topic and one could build an entire web site devoted to it. The information I've provided is a good start, but it barely scratches the surface of all there is to know about hearing loss. If you suspect your child has a hearing loss, you should definitely ask your doctor for a referral to an audiologist. Hearing loss does not have to be a barrier to education or to a happy, fulfilling life--if you are proactive about it.

Articulation skills and disorders
Your articulation skills are among the first things people notice about you; perhaps the very first, if your initial contact is by phone. It may not be fair, it may not be right, but people with articulation disorders are often negatively judged by others as soon as they open their mouths.
All young children, when they are just learning to speak, mispronounce words. As they grow older and their articulation skills develop, their pronunciation usually becomes clearer. Bilabial sounds like /b/, /p/, and /m/ are generally mastered fairly early, followed closely by /d/, /t/, and /n/. Other sounds are generally added in order of difficulty.
Most English-speaking children develop mature articulation skills and are able to pronounce all sounds accurately by about age 7 or 8. However, some children may exhibit inaccurate or distorted production of one or more sounds after age 7 or 8, and may need speech therapy to correct these distortions. If your child is receiving speech therapy for articulation, here are some articulation therapy ideas you can try at home. And here's an idea for an articulation home program to use with older children with more advanced articulation skills.
Articulation disorders in children may be developmental in nature. Their early misarticulations become habitual and do not resolve with maturation. Some medical conditions, such as hearing loss, cleft palate, or brain damage from seizures, head trauma or strokes may also cause articulation impairments. Ankyloglossia, or "tongue tie," is a condition that may interfere with articulation skills in severe cases, but not to the extent once believed to be true.
The list below shows the ages by which children generally acquire the consonant sounds of Standard American English (SAE). Many children develop these articulation skills quite a bit earlier, since there is a range of what are considered normal articulation skills. The ages below are the point at which parents should consider speech therapy if their children have not acquired the sound in question.
    3 years: p, b, m, h, w
    4 years: d, n, k, g, t, ng, f
     5 years: y
    6 years: l
     7 years: s, sh, ch, z, j, v
     8 years: r, s, th (unvoiced, as in thin, or voiced, as in these), zh (as in genre)
Also, keep in mind that girls' articulation skills tend to develop earlier and faster than boys' articulation skills.
If your child is consistently misarticulating a sound but has not yet reached the age of mastery, there are a number of additional risk factors to consider. The more of these factors your child has, the greater the risk that the distortion will not resolve itself without speech therapy.

th
Phonetic description: Interdental fricative, unvoiced (as in think, mouth) or voiced (as in these, feather).
Place of Articulation: The tip of the tongue is placed between the upper and lower front teeth; air is expelled through the mouth between the upper teeth and the tongue tip.
Common distortions: most commonly labialized as /f, v/ or stopped as /t,d/.
f, v
Phonetic description: labiodental fricative, unvoiced (/f/ as in fish) or voiced (/v/ as in very).
Place of Articulation: The lower lip is placed against the upper front teeth; air is expelled through the mouth between the upper teeth and lower lip.
Common distortions: most commonly stopped as /p, b/; less commonly backed as /s, z/.
sh, zh
Phonetic description: palatal fricative, unvoiced (/sh/ as in shoe) or voiced (/zh/ as in genre).
Place of Articulation: The tip of the tongue is raised and held close to the hard palate; air is expelled through the mouth between the hard palate and the tongue tip.
Common distortions: may be fronted as /s, z/ or stopped as /d/, but not usually as /t/. A child is more likely to pronounce shoe as doo, rather than too.
ch, j
Phonetic description: palatal affricate, unvoiced (/ch/ as in cheer) or voiced (/j/ as in jeer).
Place of Articulation: The tip of the tongue is raised to make contact with the hard palate, stopping the airflow through the mouth; as the tongue tip is lowered, air is released between the hard palate and tongue tip.
Common distortions: fronted and de-affricated as /s, z/, or stopped as /t, d/.
s, z
Phonetic description: alveolar fricative, unvoiced (/s/ as in say) or voiced (/z/ as in zebra).
Place of Articulation: Either the tongue tip or the anterior part of the tongue just behind the tip is raised and held close to the alveolar ridge (the small bump just behind the upper front teeth); air is expelled through the mouth between the alveolar ridge and tongue, then forced downward by the upper front teeth.
Common distortions: most commonly inter-dentalized as th (unvoiced or voiced), but also may be lateralized, producing a sound that has no corresponding letter in the English alphabet. Think Sylvester the Cat saying, "Sufferin' succotash!"
l
Phonetic description: alveolar lateral liquid, voiced.
Place of Articulation: The tip of the tongue is raised to press against the alveolar ridge; the sides of the tongue remain lowered, allowing air and sound to escape the mouth around the sides of the tongue.
Common distortions: most commonly glided as /w/ or as /ɣ/, formed like a /w/ except without rounding of the lips; may also be glided as /y/.
r
Phonetic description: palatal liquid, voiced.
Place of Articulation: The tongue is retracted and 'bunched' so that the main body of the tongue is below the soft palate and the tip of the tongue is below the back of the hard palate. The tip is raised so that it is just below the roof of the mouth but not making contact; the lips may be rounded and protruded, but this is not necessary acoustically.
Common distortions: may be glided as /w/, or as a more fronted /w/, almost a /v/; may also be absent following a vowel.
k, g
Phonetic description: velar stop, unvoiced (/k/ as in curl, coat) or voiced (/g/ as in girl, goat).
Place of Articulation: The rear, or base, of the tongue is raised to contact the soft palate. Air pressure builds behind the point of closure and is then released as the tongue base is lowered and the contact is broken.
Common distortions: most commonly fronted as /t, d/. The /k/ may also be glottalized in the middle of a word.
Please note that this is not an exhaustive list; any sound can be misarticulated any number of ways. I once treated a child whose articulation of /f/ involved raising the back of the tongue against the soft palate (as in the ng sound) and puffing air through the nose, so that fee sounded like hngee. This is pretty rare, however.
In addition to simple articulation errors, children may produce erroneous phonological processes. These are errors that affect entire classes of sounds rather than individual sounds.
Another disorder that affects articulation is apraxia of speech (AOS). This is a motor planning disorder that often results in inconsistent articulation of any given sound. Therapy for AOS differs from traditional articulation therapy, so it is important to have an accurate diagnosis and a therapist who knows how to plan appropriate therapy.
If a child with an articulation disorder has multiple distortions, the speech-language pathologist addressing articulation skills will usually target the earlier-developing sounds first. For example, if a child misarticulates the /k/ sound and the /s/ sound, the /k/ will be the first sound targeted because children are generally expected to master it by age 4, whereas the /s/ sound may be mastered by as late as age 8.
